Avoiding the Trap of Offense and Grievance-Mongering

Pepsi. United Airlines. Sean Spicer. At first, I thought that maybe I should give a quick re-cap of the details surrounding the recent controversies in the news related to these entities. After all, readers who missed the pertinent media reports will only perceive a random, disconnected list about as meaningful as “sketch pad, Vietnamese flag, Kim Cattrall.” Actually, I Googled Kim Cattrall and the word “controversy” just to see if there indeed ever were any controversy, and of course, there were positive hits returned. I only picked her name because I liked her in Star Trek. The Google results prove my point, though–controversy arises from everywhere these days. It really doesn’t matter in the grand scheme of things what Spicer or Cattrall actually did, and once enough time passes people will forget anyway. They’ll have moved on to the next outrage-of-the-week. What matters is that the American people, or more accurately all of us in the West, are trapped in a controversy echo-chamber. The System breeds such sensitivity into us that we’ll take offense to anything, to the point that even random lists inevitably capture examples.

People have been brought up since kindergarten to be “sensitive” and have “empathy” for anything with two legs or less anywhere in the world. As a result, we’re simply out of balance. Modern technology has caused us to forget human-scale limits. The wisest and conservative observation is this: you can’t save ’em all.

Remember that it takes two to tango. The work of transforming the words and actions of another into something offensive takes place in your own skull. The ancient Stoics knew a thing or two about this.

Often, people aren’t offended about something done or said to them–they’re third parties who observe something done to another person that makes them angry. It’s called “white knighting.” Who defines the prevailing model of morality that says the Other is always right, and that something is only good if it comes at the expense of the Self? Who does it benefit for you to feel this way? Hint: usually not you. The mainstream media curates and edits with an agenda–their agenda, not yours.

Want some mental peace? Use Dunbar’s Number–care about 150 people close to home. Senator Ben Sasse may dismiss this attitude as “tribalism,” but that’s what humans are built for. Good luck changing that. Ask yourself if there is anything about the situation in question that’s unusual for human history, considering that people have been killing each other for millennia, and will continue to do so. Ask yourself if there’s anything you can actually do about the situation, besides generate words on the Internet. Can you cook food? Deliver blankets to somewhere within driving distance? If it’s too far away to drive, forget it. Hint: attending protests to make the government “do something” doesn’t count.

I am sympathetic to the idea that some of the outrages come from the realization that when something bad happens to someone, there’s the chance in the future that it could also happen to you. This “domino theory” logic can be exhausting, though, since an infinite number of things could happen to you. Odds are, though, that these outlandish events won’t happen to you. Think of it this way: the same odds that prevent you from winning the lottery save you from dying in a terrorist attack, for example. The vast majority of the time, the supposed negative event is happening to someone else. Remember that if you’re busy looking out for someone else, then no one is looking out for you since the other guy is programmed to keep watch on events 10,000 miles away.

And if these observations on snipping the myriad cords of emotional vampirism that tie us down offend you, well, have a Pepsi.

7 Reasons Why Avoiding Unnecessary Antibiotics Is a Good Choice

Before the discovery of penicillin, no effective treatment was available for infections such as rheumatic fever, pneumonia, or gonorrhea. It wasn’t until 1928, when Alexander Fleming noticed that the “mold” growing in his petri dish was killing the Staphylococcus bacteria, that penicillin came to be. Since then, antibiotics have played a major role in conventional medicine. According to the Centers for Disease Control and Prevention, approximately 262.5 million antibiotic prescriptions are written each year in the United States. This means that more than 5 prescriptions are written for every 6 people.

However, in recent studies, it has been revealed that there is a hefty price to be paid with the inappropriate usage of antibiotics. It has been studied that bacteria have, now, been growing resistant to antibiotics that have been used time and time again. Not only that, many antibiotics are shown to have severe adverse effects, and have even led to death.

Listed below are seven reasons why avoiding unnecessary antibiotics is actually a good choice:

1. “Super bugs”.

Antibiotics have been having a harder time treating bacterial infections, such as pneumonia and staph – the very infections that were first treated by penicillin. This has shown to be linked to the increased use of antibiotics. Instead of turning to the “cure-all” drugs, try Vitamin C, which can act as an antioxidant when given in high doses.

2. Antibiotics are weakening your immune system.

Have you ever noticed that after you are prescribed an antibiotic, you just get sick again? Scientists have researched that there is a link between the prescribed antibiotics and repeat illnesses. Not only do antibiotics negatively affect the illness-causing bacteria, they also negatively affect normal cells as well. Think of it as “friendly fire”.

3. Antibiotics do not treat the cause; they suppress the symptoms.

A fever indicates that your body’s immune system is responding to an infection by producing many infection-fighting cells. Because antibiotics treat symptoms such as fever, they are actually suppressing the body’s natural defense response against toxins.

4. Some doctors are prescribing antibiotics for conditions that the medication is not intended for.

Doctors have been prescribing antibiotics for simple illnesses, such as the common cold or the flu. It has been shown the best thing to do during these types of illnesses is to let it run its course, and allow the body to naturally fight off the infection. The only contribution of the antibiotic, is that it is helping increase the resistance of the bacteria.

5. Severe allergic reactions.

Antibiotics are known to be the most common culprit of anaphylaxis, which can result in death if symptoms are not treated immediately.  These symptoms are caused by a chain reaction in the immune system. Because antibiotics are known to weaken your immune system, there is a link between the over-use of antibiotics and the development of allergies.

6. Development of Chronic Fatigue Syndrome.

A newly recognized disorder, CFS, is a result of chronic viral illness and weakened immunity. Researchers have seen that people can experience extreme fatigue and feelings of tiredness after taking antibiotics. However, they are not exactly sure why. You should ask your doctor if there is a natural alternative to treating a minor bacterial infection instead of turning to antibiotics.

7. There is a link between antibiotic use and breast cancer.

Dr. Theodoros N. Sergentanis and researchers from the University of Athens, in Greece, conducted a meta-analysis comparing the effect on breast cancer risk of individuals who have used antibiotics versus those who have never used antibiotics. They found that those who used the medications had a slightly higher risk in developing breast cancer. The link is thought to be pointed at the weakening of the immune system, and the killing of gut-friendly bacteria, due to antibiotic use.

Although antibiotics can be helpful in treating more severe bacterial infections, perhaps doctors should try and offer patients more natural alternatives to treating the less severe ailments.

Aspirin could hold key to avoiding miscarriages

Prof Simon Fishel, the care body’s managing director, mentioned he believed it could be a key trigger of recurrent miscarriage.

With appropriate therapy, he added, the variety of couples possessing wholesome infants improved to 38 per cent, a similar proportion to other infertility patients of the exact same age.

The findings were published in the journal Reproductive Biomedicine On the internet.

The genetic fault indicates the embryo is unlikely to implant in the womb or do so sufficiently, leading to late miscarriage or development problems in the infant.

If the girl is the carrier of the gene, she is also at threat of problems such as blood clots.

Prof Fishel, who led the findings, said: “Very not too long ago a new genetic marker has been discovered that predisposes couples to the danger of miscarriage, which we contact the C4/M2 variant.

“In addition to the chance of implantation failure and miscarriage, it is linked to blood- clotting problems, pre-eclampsia and reduced birth-bodyweight babies.

“What I do find outstanding is that in the population of sufferers studied, the man has the very same opportunity as the lady to pass on this variant to the building embryo and disturb productive implantation. Where the genetic variant exists, the opportunity of delivering a little one is diminished to 1 in 4 of that of fertile couples.”

Care Fertility intends to display individuals for the faulty gene so they can be treated appropriately.

A spokesman for the Miscarriage Association stated: “We constantly welcome new investigation into elements that can improve the chance of miscarriage, so we’ll be seeking closely at these findings with the support of our study skilled advisers.”

Avoiding Healthcare Cost With Pricing Transparency Hype

No less than two reports have been published final week which fall loosely underneath the banner of healthcare pricing transparency.

The 1st was Medscape’s Physician Compensation Report for 2014 (right here). The annual Medscape report is based mostly on a survey of in excess of 24,000 physicians in 25 specialties around a broad range of compensation troubles – including satisfaction.

Medscape’s findings (with data from 2013) is marginally far more insightful than the Medicare Information dump from 2 weeks in the past (here). 1 of glaring inadequacies of the Medicare data (for 2012) is the simple reality that income isn’t compensation. Seeing an annual summary of what Medicare paid to individual suppliers has practically no patient use.

MedScape2013

In accordance to the Medscape report (slides right here) there weren’t many big swings in compensation (there seldom are), but there’s usually a few noteworthy shifts. No significantly less than 19 of the 25 various practice regions noticed an improve in compensation from the prior year, 1 was neutral and five saw a decrease. Of the decreases, only 1 (nephrology) saw an 8% decline in compensation. Three of the remaining four saw a 2% decline.

All round satisfaction was close to or over 50% for most of the specialties across 4 categories (Total, Income, Career and Specialty). The two noteworthy exceptions have been Loved ones &amp Internal Medication with 32% and 27% (respectively for decision of specialty) even even though they appeared to be comparatively content (50% &amp 46%) with their income. Other findings as quoted from the slide notes:

  • Those who carry out procedures have the highest incomes compared with individuals who manage chronic illnesses. Earnings are for total-time function only. They include salary, bonus, and profit-sharing contributions. They do not consist of non-patient-connected earnings.

  • In this year’s Medscape report, the perception of being relatively compensated does not correlate to real compensation for several doctors. Orthopedists had been the most hugely paid physicians, but they fell under the middle of this record, with only 45% believing that they are fairly compensated. And despite the fact that plastic surgeons were seventh between the leading earners, at 37% they have been the least most likely of all physicians to think that they are fairly paid.

  • When evaluating newer payment versions with survey results from 2011, there has been a dramatic rise in doctors choosing ACOs, with 24% of all doctors now employed in these organizations vs only 3% in 2011. Moreover, ten% are planning on joining this yr. In spite of considerable publicity, income-only and concierge practices are nonetheless not significant payment designs.

  • 5% of employed and 15% of self-employed doctors explained that they will not get new Medicare or Medicaid patients.

  • Although Medicare is known to have lowered fees, private insurers are also paying less, especially to modest practices without having any strong influence. Robert Morrow, MD, a family members physician in the Bronx and a Medscape advisor, mentioned he now receives $ 82 from Medicare for an office visit but only about $ 45 from commercial insurers.

  • About 72% of respondents to this survey routinely or sometimes go over the expense of treatment method with individuals. This has gone up from 68% in 2012.

  • 35% of employed doctors devote at least ten hrs a week on paperwork compared with 26% of the self-employed.

The second report – Price Transparency in Overall health Care (PDF right here) – was from the Healthcare Financial Management Association (HFMA). For people unfamiliar with HFMA, their site (right here) describes the organization this way:

HFMA is the nation’s top membership organization for healthcare monetary management executives and leaders. More than forty,000 members-ranging from CFOs to controllers to accountants-consider HFMA a respected believed leader on best trends and troubles dealing with the healthcare industry.

In fairness, they exclusively indicate that the “primary audience for this report is sector stakeholders in provider, payer, and purchaser settings that this report calls upon to take distinct actions to improve the transparency of healthcare prices” (ie: not customers or sufferers).

The conclusion from HFMA’s 20-page report is just this:

The lack of price transparency in well being care threatens to erode public believe in in our healthcare system, but this erosion can be stopped. Sufferers are assuming greater financial accountability for their healthcare needs and in turn need the data that will permit them to make informed healthcare selections. Price is not the only details needed to make these selections as this report has mentioned, cost must be presented in the context of other relevant data on the quality of care. But it is an important element. The time for price transparency in well being care is now.

I have grave doubts about stopping the erosion of public trust. Two of the seven headings are “Guiding Ideas for Value Transparency” and “Recommendations for Price tag Transparency Frameworks.” In impact, it’s generally another market “call to action.”

Sadly, the worth of any of this “transparency” is truly marginal – either in terms of our own actions – or in terms of the influence on our Nationwide Healthcare Expenditure (NHE). The Centers for Medicare and Medicaid (CMS) forecasts our NHE to be $ 3.1 trillion this 12 months. In reality, our NHE as forecasted by CMS (Table one web page five right here) seems to be like this through 2022 (the two with and without Obamacare).

ACANHEForbes1

All of which tends to make this type of transparency comparable to rubbernecking a freeway accident. There’s a short minute of anxiousness and tension and then everyone speeds on their way. Assuming data accuracy, releasing any healthcare pricing is a hollow objective because there is virtually nothing at all any person can do with it. We merely have no way to influence it – either individually or collectively – and absent quality (or clinical outcomes), there is no way to equate any worth. We may possibly as properly use random amount generators.

The 1 point it constantly does, nonetheless, is to highlight how significantly our healthcare program is optimized all around income and earnings. Possibly someday this will translate into a debate about real healthcare reform – the one we continually keep away from (cough – SGR / “doc fix” – cough). That debate is not called transparent anything. Its known as healthcare cost.

Avoiding HIV: the most current on vaccine improvement and gene therapy

There has been a advancement in HIV prevention and treatment. What’s occurred?

Two groups in the US [the Aaron Diamond Aids analysis centre and the Centre for ailment handle and prevention] have shown that injection of prolonged-acting HIV drugs could totally avoid infection of monkeys. This is significant since, if it operates in people, it may be feasible to get a shot in the arm every 3 months to protect by yourself against HIV. The exact same technological innovation could be employed to treat individuals presently infected with the virus.

Will the new advances in gene therapy influence therapy in a comparable way and how do they perform?

Despite the fact that innovative, these findings are at a ‘proof of concept’ stage. Scientists had been ready to acquire massive numbers of white blood cells from infected individuals, and use gene treatment to eliminate a single of the receptors that lead to HIV infection. These cells had been then transfused back into the sufferers and remained resistant to infection. Although promising, only a tiny percentage of cells actually grew to become resistant. A lot of work will be needed to make this technique clinically effective. The very likely cost of this kind of an method in its recent format tends to make it an unrealisable option for people in the building globe. If the technological innovation progresses, this may possibly sooner or later adjust but at the moment this is an aspirational purpose.

You specialise in HIV transmission what are you operating on at the minute?

At the Imperial College London department of medication, we are operating on establishing preventive vaccines towards HIV and repurposing the use of drugs typically utilized in treatment, to avert infection. It is an fascinating time as the two fields are moving quite rapidly.

Anti-retroviral drugs support maintain infections down, but the far more they are utilised the more drug-resistance goes up. Will this new development assist get close to this problem?

Drug resistance transpires largely when patients miss taking their medicines. Prolonged-acting, injectable medicines will get about this issue as individuals won’t need to don’t forget to get tablets each day. It would also be useful for prevention as it could be administered along with injectable forms of prolonged-acting contraception.

What ought to different groups in the advancement sector do next to make the most of this new study?

This is a single of a range of new technologies that may possibly aid reduce the epidemic. To make the most of this and other emerging approaches, human trials will need to have to be accelerated. We have to be sure that any approach is risk-free, effective, inexpensive, accessible and wished by those most at danger of infection. There is no level in creating something that no person can afford or will want to use. This can only be completed by involving broad groups from the development sector to target affected communities. In the meantime, we want to encourage strategies that we know decrease transmission, which includes condom promotion and secure sex practices.
