Antibiotics overuse could increase bowel cancer risk, study finds

The overuse of antibiotics could increase a person’s risk of developing bowel cancer, the findings of a US study suggest.


Research published in medical journal Gut found extended use of antibiotics significantly increased the chance of polyp formation in the colon, a precursor of bowel cancer.


The findings add weight to emerging evidence that the type and diversity of bacteria in the gut may have a key role in the development of cancer, according to the authors of the study.


An Australian bowel cancer expert, Associate Professor Graham Newstead, the head of the colorectal unit at the Prince of Wales private hospital and director of Bowel Cancer Australia, said the research had “credence”.


“We know already that if you take antibiotics you frequently get diarrhoea,” Newstead said.


This happened because the antibiotic killed some of the normal bacteria, leading to an overgrowth of the abnormal bacteria in the gut.


But Newstead said the study did not look at the effect of antibiotics on the colon and caution must be taken.


US researchers monitored the health of more than 120,000 nurses, aged between 30 and 55, who were part of the the Nurses Health Study launched in 1976.


Between 2004 and 2010, 1,194 abnormal growths in the colon and rectum were diagnosed.


Those who had taken antibiotics for two months or more between the ages of 20 and 39 were 36% more likely to be diagnosed with an adenoma, or polyp, compared with those who had not taken antibiotics for any extended period in their 20s and 30s.


Similarly, women who had taken antibiotics for two months or more during their 40s and 50s were 69% more likely to be diagnosed with an adenoma.


And those who had taken these drugs for 15 days or more between the ages of 20 and 39, and between the ages of 40 and 59, were 73% more likely to be diagnosed with an adenoma.


“It does seem to indicate that people who have too many antibiotics might be at more risk of of getting polyps than people who have less of them,” Newstead said.


“And, remembering that not all polyps turn to cancer but the cancer comes from the polyps. If you have more polyps or tendency to get polyps then maybe you are slightly more at risk of getting cancer.”


The message to be taken from the study was not to use antibiotics for a “tickle” in the throat or a cold, Newstead said.

Antibiotics could be an effective alternative to surgery for treating children with appendicitis, research suggests.


According to the NHS, appendicitis affects an estimated one in 13 people at some point in their life, with appendix removal the most common reason for emergency surgery in children.


But researchers say using antibiotics alone might offer a less invasive alternative – an approach that has already had some success in adult patients.


“It has become clear in recent years that in adults there are some patients with appendicitis who can recover from the disease without an operation, and we are frequently asked by parents of children with an appendicitis whether their child really needs an operation to get better,” said Nigel Hall, associate professor of paediatric surgery at the University of Southampton and co-author of the study.


While surgery remains the “gold standard” tried and tested treatment, often with some antibiotics, said Hall, “we are keen to explore the role of non-operative treatment for these children.”


Published in the journal Pediatrics by a team of scientists from the UK and Canada, the research involved a review of 10 existing studies published within the last decade. In total the studies involved 766 children from countries around the world – but not including the UK – of whom 413 were treated for uncomplicated, acute appendicitis with antibiotics alone rather than surgery.


Six of the studies compared the use of antibiotics alone with surgery, while the other four looked only at results for children treated with antibiotics. Different antibiotics and course durations were used in the studies, with both intravenous and oral regimes employed.


Overall, the authors found that the use of antibiotics alone was effective for 97% of children undergoing non-surgical treatment, while none of the studies reported any adverse effects of treating appendicitis with antibiotics.


However, appendicitis recurred in 14% of the children who did not have surgery. Overall, 82% of children who were treated with antibiotics alone avoided having surgery by the end of the various studies, a period that ranged from eight weeks to four years, depending on the research.


“While the benefit of non-operative treatment might be that you can avoid an operation, if you get a recurrence of your appendicitis it is likely that you will then be recommended to have an operation to remove the appendix in the long run,” said Hall. “So the benefit of avoiding an operation in the short term is gone.”


With only one of the ten studies considered in the review a randomised control trial, further rigorous research is needed to compare the effectiveness of antibiotics alone versus surgery for appendicitis, as well as to evaluate costs and quality of life for the different treatments.


“We really do need to do the prospective, comparative, randomised studies in order to be able to find out which is the better treatment option,” said Hall, adding that initial steps towards such studies are already underway in the UK by a team including Hall.


Furthermore, he noted, the review only covers simple, acute appendicitis and does not include children with complicated appendicitis, such as those with a perforated appendix or an appendix mass.


“We would very much not recommend that all children with appendicitis are treated with antibiotics and also very much that treatment of appendicitis remains a condition that needs to be treated by a specialist surgeon in a hospital,” said Hall.


Anthony Lander, a consultant surgeon at Birmingham Women’s and Children’s Hospital who was not involved in the study, acknowledged that both surgery and antibiotics alone have their benefits and drawbacks.


“Operations are expensive and have complications but there is no recurrent appendicitis. Antibiotics are cheap and very safe but may fail to treat the illness and surgery may still be required,” he said.


While Lander agreed with the need for large, rigorous studies to compare which approach might be best for treating early acute appendicitis, he added that the condition differs from person to person.


“A more valuable question would be “ What is the nature of the appendicitis which can be treated by antibiotics alone, safely and without a high recurrence rate?” and “Which cases should have an operation?”,” he said “The challenge is to design a study to answer this question.”.


John Abercrombie, a consultant colorectal surgeon and spokesperson for emergency general surgery at the Royal College of Surgeons, welcomed the study. “What this shows is quite similar to similar publications looking at appendicitis in adults where antibiotic treatments are being shown to be safe for selected cases,” he said.


“It is an important treatment option that needs to be discussed and for suitable children it seems like a very reasonable thing to do and may offer the chance to avoid an operation,” he added.

Antibiotics leave children "more likely to contract drug-resistant infections"

Children are at substantially increased risk of contracting drug-resistant infections in the months after taking a course of antibiotics, a leading public health official has warned.


Paul Cosford, medical director at Public Health England, told MPs on Wednesday that children are 12 times more likely to contract drug-resistant infections in the three months after being prescribed antibiotics, suggesting that their unnecessary use poses a direct risk to individual patients as well as a broader threat to society as a whole.


“We’ve got good evidence that if you or I have a course of antibiotics now, within three months our risk is three times to get a resistant infection of some sort because we’ve had the antibiotics affecting all the organisms in our bodies,” he told the Science and Technology Select Committee. “If you’re a child you’re 12 times more likely to get a resistant infection in the three months after a course of antibiotics.”


He said the figures, based on two major reviews, highlighted the need to continue driving down our reliance on the drugs. “There is a growing body of evidence that taking antibiotics makes it more likely that your next infection will be a resistant one, so prudent use of these life-saving medicines is essential,” he told the Guardian.


The study cited by Cosford, in fact found that children who had urinary tract infections (cystitis) were 13.23 times as likely to have contracted drug-resistant strains if they had been given antibiotics in the previous six months.


In a previous survey, 90% of GPs said they come under pressure from patients to hand out the antibacterial medication, and 45% said they had done so knowing it would not help.


Mark Woolhouse, professor of infectious disease at the University of Edinburgh, said that the figures matched his qualitative experience from the clinic. “In hospitals you definitely do get patients who will get a string of different drug-resistant infections,” he said.


Antibiotics pose a risk of subsequent illness, he said, because the drugs do not uniquely target the infection, but also eradicate useful bacteria in the gut. “They change the ecology of the gut, a bit like using a pesticide in a rich woodland,” he said. In the ecological niche that opens up after a course of antibiotics, opportunistic infections can spring up.


“This comes with the proviso that people should follow their doctor’s advice,” he added. “Antibiotics are a good thing when prescribed correctly, but they can have longer-term disadvantages of all kinds.”


The committee heard that that overall Britain had made progress on reducing the use of antibiotics since the government asked the economist Jim O’Neill to produce a report on antimicrobial resistance two years ago.


Prescriptions of antibiotics fell by 7.3% in the financial year 2015-16 compared with the previous year, Cosford told the committee.


Farms are also on track to meet a Defra target for 2018 of reducing average antibiotic use on farms to 50mg of drug per 1kg of meat produced (56mg this year, compared to 62mg in 2014). The target is designed to combat the blanket use of antibiotics as a way of boosting the growth of livestock – particularly poultry and pigs.


Certain strains of drug resistant infections are decreasing, the committee heard, with only 800 cases of MRSA bloodstream infections last year, compared to 7,000 in 2003. “When we focus on specific infections that are causing a problem then we can be extremely successful,” said Cosford. However, a more worrying trend was the rise in drug resistant E coli strains, he said, mostly linked to urinary tract infections.


O’Neill said that the current government appeared “less passionate” about the issue than the previous administration. “There is interest, perhaps not with the same passion as with David Cameron,” he said. “The prime minister himself and the chancellor played an active role in international engagement on the issue. I have not seen this government talk about it anything like as much as the previous leadership did.”


He added that it was crucial for policy-makers to make international efforts now rather than waiting for the “scary outbreak of a crisis”.


“We need to stop treating these things like sweets,” O’Neill said. “We all need to be re-educated that these things aren’t a magical solution for as many things as people think.”

Experts raise alarm over declining effect of last-line antibiotics

Last-line antibiotics against serious pneumonia and bloodstream infections are under real threat in Europe as resistant strains of bacteria emerge, experts are warning.


A new report from the European Centre for Disease Prevention and Control (ECDC) reveals that resistance has continued to increase across Europe in spite of attempts to raise global awareness of the danger to the fundamentally important antibiotic class of drugs. Without them, some infectious diseases could become untreatable and some forms of major surgery would again become perilous.


The report shows that antibiotic resistance continued to increase between 2012 and 2015 across the EU for most of the bacteria and drugs that are being monitored.


Particularly worrying is the increase in resistance to antibiotics considered the last line, where there are no new drugs to treat patients with certain serious infections.


In the Klebsiella bacteria, which is responsible for dangerous pneumonia infections, resistance to the antibiotic class called the carbapenems increased from 6.2% in 2012 to 8.1% in 2015. Combined resistance to carbapenems and polymyxins, such as the antibiotic colistin, was reported in some countries.


Antibiotic use went up in hospitals across the EU, although it went down in community settings in six countries.


Vytenis Andriukaitis, the European commissioner for health and food safety, said: “Antibiotic resistance is one of the most pressing public health issues of our time. If we don’t tackle it, we can go back to a time when even the simplest medical operations were not possible and organ transplants, cancer chemotherapy or intensive care even less so.


“The European commission will launch a new action plan next year so that we can, together with our partners in the EU member states and internationally, continue to ensure that the prevention and control of antibiotic resistance is strengthened within a one-health approach.”


There are concerns about the rise of strains of E coli that are resistant to antibiotics. The bacteria is a frequent cause of infections to the bloodstream and urinary tract, which could become harder to treat.


Widespread publicity about the dangers of the “superbug” MRSA (methicillin-resistant staphylococcus aureus) and efforts to increase hygiene in hospitals brought the levels down between 2012 and 2015, but eight out of 30 countries still have levels of resistance of 25%, making it a public health priority.


Dr Andrea Ammon, the acting director of the ECDC, said there was a lot of variation in the picture around Europe, but generally the proportions of resistant bacteria were higher in the southern and southeastern European countries.


“Multidrug resistance is worrying,” she said. “We also have some reports of combined resistance against so-called last line antibiotics.”


These drugs, which include the carbapenems and colistin, are usually kept in reserve for infections that other drugs can no longer treat.


There have been single reports of resistance to the last-line antibiotic colistin in the Czech Republic, France, Germany, Greece, Italy, Poland and Romania.


“It is not a large problem but it exists. Whenever we have something like this and don’t take good care, it spreads,” she said.


New classes of antibiotic drug are needed, butAmmon said: “We know what can be done and what should be done right now regardless of what new drugs come along. We must try to do the utmost to make the current antibiotics last as long as possible.”


That meant using them prudently and only when necessary and at the right dosage, as well as careful hand hygiene control, especially in hospitals.

The development of antibiotics for treating bacterial infections has indeed lengthened human lifespan. However, excessive and inappropriate use of these useful drugs may cause serious long-term consequences, which not only affect our individual health but may even cause permanent changes particularly to the gut microbiota of our descendants for generations.


Maintaining a proper balance of healthy gut microbiota is crucial as it plays an essential role in metabolism, detoxification, nutrition, development, pathogen resistance, and regulation of immune responses.


Microbiota has been used by scientists to replace the term microflora, which refers to symbiotic and pathogenic microorganisms comprise bacteriafungi, and archaea.

Disharmony


Studies have shown that antibiotics may be associated with disharmony or imbalance of gut microbiota, which in turn impacts its activity that leads to the development of various diseases.


One study found that a single intravenous dose of antibiotic demonstrated a significant change in the variety bacterial strains of fecal bacteria and caused the emergence of Clostridium difficile. Another study demonstrated that a short course of the antibiotic ciprofloxacin  treatment influenced the abundance of roughly one-third of  genetically distinct types of bacterial population in the gut, decreasing diversity, the richness, and evenness of the community.  The study also found that much of the bacteria recovered by four weeks after the end of treatment, but there were several species failed to recover within 6 months. The result suggests that a short course antibiotics may cause prolonged changes in the diversity of gut microbiota.


Most of the cases of antibiotic-associated diarrhea (AAD) are presumedly related to distinct changes in the diversity of the gut microbiota. Changes in the gut microbiota ecosystem allow pathogens such as C. difficile to proliferate. Staphylococcus aureus, Clostridium perfringens, and Klebsiella oxytoca are also identified as potential causes of AAD.


A recent study demonstrated that salmonella was able to flourish in the gut after antibiotic treatment because of the increased availability of oxidized sugars. The study suggests antibiotics increase the availability of nutrients in the gut, enabling growth of pathogens.


Dysbiosis


Antibiotic treatment is also associated with gut inflammatory diseases including inflammatory bowel disease and Crohn’s disease. Antibiotic use in the first year of life is associated asthma, eczema, and rhinoconjunctivitis. Antibiotic use during pregnancy and early life have been associated with increased risk of food allergies. The risk increases with the increasing number of antibiotic courses.


Another study showed that antibiotic use in early life increases the risk of overweight in later life. Fecal microbiota composition in infancy may predict whether a child will be overweight or normal weight at age seven.


The most crucial step towards reducing antibiotic use is preventing the need for them in the first place. Asking doctor to determine that the condition is bacterial in origin is a good start. Besides that, ask whether the condition requires antibiotics or it is self-limiting.

Use of strongest antibiotics rises to record levels on European farms

Use of some of the strongest antibiotics available to treat life-threatening infections has risen to record levels on European farms, new data shows.


The report reinforces concerns about the overuse of antibiotics on farms, following revelations from the Guardian of the presence of the superbug MRSA in UK-produced meat, in imported meat for sale in UK supermarkets, and on British farms.


According to the data from the European Medicines Agency, medicines classified as “critically important in human medicine” by the World Health Organisation appear to be in frequent use on farm animals across the major countries of the EU, including the UK. This comes in spite of WHO advice that, because of their importance, these drugs should be used only in the most extreme cases, if at all, in treating animals.


The latest report from the EMA collates data from member states on the sales of antibiotics for veterinary purposes in 2014, and shows that antibiotic use on farms fell by about 2% on the previous year overall, and by as much as 12% in many countries. But this disguises the rise in the use of the strongest medicines, such as colistin, which is a last resort for life-threatening human illness.


The percentage of antibiotics sales made up by the most potent antibiotics remained steady or in some cases increased slightly, indicating an increase in the amount of so-called critically important antibiotics used.


For instance, sales of fluoroquinolones – the newest versions of which are used to treat life-threatening illnesses including pneumonia and Legionnaire’s disease – stood at 141 tonnes across the countries surveyed in 2013, and rose to 172 tonnes in 2014. Sales of macrolides, also classed as critically important to human health, rose from 59 to 67 tonnes in the same period. This shows that efforts to prevent the drugs most crucial for human health from being used in farming are failing.


Experts are increasingly concerned by growing evidence that the overuse of antibiotics on farms – which in the EU account for three times the quantity of antibiotics dispensed to the human population – is endangering human health by fostering the development of bacteria resistant to even the strongest medicines.


Cóilín Nunan, of the Alliance to Save Our Antibiotics, which comprises several NGOs concerned with animal and human health, said: “The shocking overuse of farm antibiotics shown [in the report] is a result of the continued failure by most countries to ban routine preventative mass medication in intensive farming.


“Spain now uses 100 times more antibiotics per unit of livestock than Norway, 80 times more than Iceland and 35 times more than Sweden. The main reason for the difference is that Spain, like most of Europe, allows routine mass medication, whereas the Nordic countries do not. Superbugs: resistance is not futile but a new generation of antibiotics is necessary

Meh. What is so fantastic about that? Practically nothing. Superbugs could send us “back into the dark ages”, according to England’s chief health-related officer.
