As I lie back across the thin blue paper runway, my legs open, an Anglepoise lamp shining into the abyss, my nurse (called, ironically, Comfort) warns: “This may be a little uncomfortable.” Oh how we laughed. But I was determined – I wanted a coil.


According to reports, in the first week after the US election, Planned Parenthood (which Donald Trump, vice president Mike Pence, and House speaker Paul Ryan all want to stop funding, by the way) saw a 900% increase in patients seeking IUDs. As the news that Trump was to nominate “women’s health opponent” Tom Price for secretary of health and human services fell across America like sleet, women, nurses and healthworkers took to social media advising one simple thing: get a coil.


It builds on an existing trend for American women to choose intrauterine devices. Under Barack Obama’s Affordable Care Act, insurance providers are required to cover a woman’s birth-control method of choice for free; usually this costs about $ 1,000 for insertion and follow-up visits. For the first time, the IUD was affordable, as well as reliable. And so, according to US Center for Disease Control and Prevention’s National Center for Health Statistics (NCHS), IUD use increased 83% comparing 2006–2010 with 2011–2013. That the source of my sadness could be linked to the galloping horse-sized quantity of hormones running through my bloodstream didn’t occur to me for longer than I care to admit. But when it did, the solution seemed as clear as it did to that tweeting nurse in Colorado: get your IUD.


As Vicky Spratt of The Debrief has written for their new Mad About The Pill campaign, the link between the contraceptive pill and mental health problems is still just anecdotal. But by god the anecdotes are there. According to the site’s survey of 1,022 women, 46% reported that the pill had decreased their sex drive, 45% said that they believed they had experienced anxiety, 45% said they had experienced depression and 20% reported experiencing panic attacks which they attributed to their hormonal contraception. If we were in America, we’d have had to pay for the pleasure of this troublesome medicine. So no wonder so many of us are looking into alternatives.


The copper coil didn’t make me weep uncontrollably with the curtains closed, it didn’t turn my tits into bricks and I never had to panic that I’d left it at home. You do not forget about your coil and then have to seek emergency contraception; you can keep the same coil in for up to 10 years, which means you can be miles from a chemist, in another country, or simply at work and never have to worry. It will not tear while fumbling in the gloom of your parents’ spare room; it won’t get punctured by the keys in your pocket; you don’t have to pay £2 in a pub toilet for it and, in my case at least, it didn’t hurt.


Coils give us hormone-free control over our wombs and, therefore, lives. On the downside they do nothing to protect against STIs and there are, of course, women who suffer serious side-effects – heavy or irregular periods, damage to the womb, pelvic infections, ectopic pregnancies. But the coil still deserves to move away from the icky, scary reputation it had when our mothers were sleeping around.


Of course I find it exasperating, frustrating, maddening and saddening that the burden of not getting pregnant still falls almost entirely on women. Many men will still assume that she “has it covered”; many still squirm away from condoms; some seem entirely unaware of what sperm may do. It’s 2017, for Christ’s sake – and yet, I know firsthand that many women are expected to be magically infertile right up until the moment their partner wants a baby. And, should that woman want to get pregnant earlier? Well then, of course, the question of coming off contraception must be a joint one. He must have his say. If he’s not ready then you cannot push it on him. If you do, you are branded selfish and controlling.


When it comes to our bodies, we carry all the responsibility; but not quite all of the power. But, perhaps, the coil can help claim some back.

How KFC, Subway and McDonald’s can help the fight against antibiotic resistance | Letters

By 2050, drug-resistant infections are expected to cause 10 million deaths annually – becoming a bigger killer than cancer is today. By 2050, antimicrobial resistance is also expected to cost the world $ 100tn and could push more than 28 million people into extreme poverty.


Misuse of antibiotics in food animals is a major driver of resistance. Farm animals consume about two-thirds of the world’s antibiotics, with much of this added to feed or water to make animals grow faster or to counter unsanitary conditions in factory farming facilities. Between 2010 and 2030, it is predicted that antibiotic use in food animal production will increase by two-thirds.


In World Antibiotics Awareness Week, we are calling on KFC, Subway and McDonald’s to end the routine use of all antibiotics included on the World Health Organisation’s list of medically important antimicrobials, in all of their livestock supply chains. This means prohibiting suppliers from using these antibiotics for growth promotion or disease prevention and only using them when there has been a diagnosis of illness.


We welcome the progress that has been made by Subway and McDonald’s in North America and urge KFC to follow suit. But action in one region will not be enough. Consumers worldwide are becoming increasingly aware of the negative health impact of misuse of antibiotics in livestock farming. Drug-resistant infections do not respect national boundaries. We urge KFC, Subway and McDonald’s to make global commitments and develop timetables for action with targets.

UN agrees to fight "the biggest threat to modern medicine": antibiotic resistance

All 193 United Nations member states are set to sign a declaration agreeing to combat “the biggest threat to modern medicine” in Wednesday’s high-level meeting on antibiotic resistance.


The agreement was reached just before the general assembly convened to discuss the threat of antibiotic resistance, which is only the fourth health issue to trigger a general assembly meeting.


“It’s ironic that such a small thing is causing such an enormous public threat,” said Jeffrey LeJeune, a professor and head of the food animal research program at Ohio State University. “But it is a global health threat that needs a global response.”


The declaration routes the global response to superbugs along a similar path as the one used to combat climate change. In two years, groups including UN agencies will provide an update on the superbug fight to the UN secretary general.


It is estimated that more than 700,000 people die each year due to drug-resistant infections, though it could be much higher because there is no global system to monitor these deaths. And there has been trouble tracking those deaths in places where they are monitored, like in the US, where tens of thousands of deaths have not been attributed to superbugs, according to a Reuters investigation.


Scientists warned about the threat of antibiotic resistance decades ago, when pharmaceutical companies began the industrial production of medicine. The inventor of penicillin, Alexander Fleming, cautioned of the impending crisis while accepting his Nobel prize in 1945: “There is the danger that the ignorant man may easily underdose himself and by exposing his microbes to non-lethal quantities of the drug make them resistant”.


But in the last few years, studies have dramatically increased awareness about antibiotic resistance. There has also been considerable advocacy by health officials, like Sally Davies, chief medical officer of the UK.


“Drug-resistant infections are firmly on the global agenda but now the real work begins,” Davies said in a statement. “We need governments, the pharmaceutical industry, health professionals and the agricultural sector to follow through on their commitments to save modern medicine.”


Signatories to the UN declaration committed to encouraging innovation in antibiotic development, increasing public awareness of the threat and developing surveillance and regulatory systems on the use and sales of antimicrobial medicine for humans and animals.


Only three other health issues have been the subject of general assembly high-level meetings: HIV/Aids, non-communicable diseases and Ebola.


Mark Woolhouse, professor of infectious disease epidemiology at the University of Edinburgh, said he was encouraged that unlike with HIV/Aids and Ebola, the UN is addressing this health crisis before it has spun out of control.


“It’s very serious indeed – it’s killing people around the world at the rate of hundreds of thousands of year and we all expect it to get worse if something isn’t done now,” Woolhouse said. “But the UN is coming in at just the right time, in a sense.”

Scientists have warned that potentially deadly fungal infections are acquiring resistance to many of the medicines currently used to combat them. More than a million people die of fungal infections every year, including about 7,000 in the UK, and deaths are likely to increase as resistance continues to rise.


Researchers say the widespread use of fungicides on crops is one of the main causes of the rise in fungal resistance, which mirrors the rise of resistance to antibiotics used to treat bacterial infections in humans.


“There are close parallels between bacterial and fungal resistance, though the problems we face with the latter are particularly worrying,” said Prof Adilia Warris, a co-director of the newly opened Centre for Medical Mycology at Aberdeen University.


“There are more than 20 different classes of antibacterial agents. By contrast, there are only four classes of anti-fungal agents. Our armoury for dealing with deadly fungi is much smaller than the one we have for dealing with bacteria.


“We cannot afford to lose the few drugs we have – particularly as very little funding is being made available for research into fungi and fungal infections.”


Fungi cause a range of illnesses – such as thrush, athlete’s foot and dandruff – that can be treated relatively easily.


Other illnesses have more serious consequences. Individuals who are receiving bone marrow transplants and who are immune-suppressed can die of aspergillus and candida fungi infections, for example.


Another example of their grim potential was highlighted last week when doctors reported that a bagpipe player had died because deadly fungi had infected his pipes.

“Fungi are everywhere,” said Prof Gordon Brown, head of the Aberdeen mycology centre.


“We breathe in more than 100 spores of aspergillus every day. Normally our immune systems mop them up but, when our disease defences are compromised – for example, during cancer treatments or after traumatic injuries – they lose the ability to fight back.


“Fungi can spread through patients’ bodies and into their spines and brains. Patients who would otherwise survive treatments are dying every year from such infections.”


This point was also stressed by Prof Neil Gow, another Aberdeen researcher. “Essentially fatal fungal infections are diseases of the diseased,” he said.


In addition, premature babies and patients with the inherited condition cystic fibrosis are also vulnerable.


However, the problem is even worse in developing countries. In sub-Saharan nations, where millions are infected with HIV – which causes severe depletion of patients’ immune systems – infections with cryptococcus and pneumocystis fungi account for more than half a million deaths a year.


“The total global number of fungal deaths is about the same as the number of deaths from malaria but the amount that is spent on fungal infection research is only a fraction of the cash that goes on malaria research,” added Gow.


A vaccine that could protect against fungal disease has yet to be developed, while the rise of resistance to the class of medicines known as azole drugs is causing alarm among doctors.


Recent reports from the US and Europe indicate that resistance to azole drugs is increasing in both aspergillus and candida fungi. The widespread use of agricultural fungicides to protect crops and their use in some paints and coatings has been linked to the rise of this resistance.


Doctors have recently uncovered another worrying development: outbreaks of fungal infections – mainly cryptococcus – that have appeared in previously healthy people. In one outbreak, in the northwest US, dozens of people died.


In the wake of these developments, it was decided by Britain’s Medical Research Council to open its Aberdeen mycology centre earlier this year.


It will employ experts in the field to gain new understanding of how fungi move into the human body and survive there. It will also work on the development of new drugs and tests for pinpointing specific fungi that are infecting patients.


“Fungal infections are going to be an increasing problem in coming years and we need to develop the best defences,” said Brown. “We aim to do that here.”
