Baby boomers warned over alcohol intake as hospital admissions soar

Alcohol-related hospital admissions in England have increased by 64% in a decade and are at their highest ever level, prompting experts to warn that baby boomers are continuing to risk their health through frequent and excessive drinking.


There were an estimated 1.1m admissions where alcohol was the primary or secondary issue in 2015/16, compared with 670,000 in 2005/06, according to NHS Digital data published on Wednesday.


Separate data from the Office for National Statistics (ONS) for England shows people aged 45 and over are frequently drinking at more hazardous levels.


Married and cohabiting couples are more likely than single people to consume alcohol on five or more days a week, though they are slightly less likely to binge drink.


Surveys found 60% of women aged 45 to 64 and 69% of men of the same age had drunk alcohol in the last week – the highest proportions of any age group.


Prof Sir Ian Gilmore, a liver doctor and chair of the Alcohol Health Alliance UK, said the figures proved the UK continued to have a dysfunctional relationship with alcohol.


He said: “We know that over the long term, rates of binge drinking are falling and more people are choosing to abstain from alcohol. Worryingly, however, these trends do not appear big enough to stop alcohol harm from continuing to rise, and the sharp increase in alcohol-related hospital admissions over the last few years means hundreds of thousands more people each year are experiencing the misery associated with harmful alcohol consumption.”


Dr Tony Rao, co-chair of the Older People’s Substance Misuse working group at the Royal College of Psychiatrists, said: “These figures show that alcohol abuse is not a ‘young person problem’. While the rest of the population reduces its alcohol intake, it is very concerning that baby boomers are drinking at a similar rate as before and are exceeding recommended guidelines.”

The ONS data also shows that the proportion of adults who say they drink alcohol is at the lowest level for more than a decade.


The trend has been largely driven by an increase in the proportion of younger people abstaining, but there has been no decline in the proportion of over-65s drinking, and they are the age group most likely to have consumed alcohol on five or more occasions in the week before they were interviewed.


Such regular drinking is more than three time more likely among over-65s than in the 16-24 age group, the data shows.


Alcohol is linked to more than 60 illnesses and diseases including heart disease, liver disease, cancer and dementia. Hospital admissions due to liver disease have gone up 57% over the last decade, and the number of people diagnosed with alcohol-related cancer has risen by 8%, according to the AHA. The World Cancer Research Fund has said that if nobody drank alcohol in the UK, 21,000 cases of cancer could be prevented each year, including nearly 12,000 cases of breast cancer.


NHS Digital cautions that a narrower measure of alcohol-related hospital admissions is more reliable for tracking changes over time than the broad measure – which the 1.1 million figure relates to – because it is less affected by improvements in recording of secondary diagnoses. But even under the narrow measure, hospital admissions have risen by 22% since 2005/06, to 339,000.


The ONS data shows that 7.8 million people admit to binge drinking – defined for men as consuming more than eight units and for women more than six units – on their heaviest drinking day.


Men are more likely to drink than women and to binge drink. Higher earners, on £40,000 and above annually, are more likely to be frequent drinkers and to binge drink than the lowest earners.


Gilmore said: “The data released today should be sobering reading for whoever wins the upcoming general election, and we would urge the next government to make tackling alcohol harm an immediate priority to save lives, reduce harm, and reduce the pressure on the NHS.”


The Alcohol Information Partnership, which is funded by global drinks companies, said binge drinking and harmful drinking had declined by 17% and 23% respectively since 2005.

“We have identified a number of areas of concern, which highlight potential inadequacies in the level of care and support this group is receiving outside the emergency hospital setting”, said co-authors Ellis Keeble and Lucia Kossarova, who work at the Nuffield Trust thinktank.


They undertook the research, alongside the Health Foundation thinktank, by examining official NHS hospital episodes statistics records for admissions to hospital in England by newborns up to those aged 24 between 2006-07 and 2015-16.


Emergency hospital admissions for all under-25s grew during the decade from 990,903 in 2006-07 to 1,124,863 in 2015-16 – a rise of 14%. However, that was lower than the 20% increase in emergency admissions among the population as a whole over that time.


But they learned that there had been a 30% jump in the number of infants having to be admitted and a 28% rise among those aged one to four. “The number of infants, children and young people using emergency departments continues to increase – with 30% more emergency admissions for under ones and 28% more for one- to four-year-olds than 10 years ago. This is placing huge pressure on a service already under strain,” said Dr John Criddle, a spokesman for the Royal College of Paediatrics and Child Health.


The number of admissions among infants for jaundice more than doubled from 8,186 to 16,491 per year and cases of acute bronchitis rose 88% from 21,235 to 39,122.


Spells in hospital for “other perinatal conditions”, which includes digestive problems and feeding trouble in babies, also increased sharply, from 14,293 to 24,848. Upper respiratory infections except asthma rose from 17,357 to 20,940; viral infections rose by 116% from 12,730 to 16,826.

Dr Bob Klaber, a paediatrician at St Mary’s hospital in London, said: “There is no one medical explanation for why 30% more young children are now being admitted to hospital than before. So there must be something about how we are supporting these families and a lack of support for mums with newborns who are socially isolated.


“From my experience, the fragmentation of our health and social care system, including antenatal support and help in the child’s first five years of life, has made things really difficult for parents with new babies and young children, and so too often our hospitals become the default place for them to attend.”


A lack of focus on developing joined-up preventative care plans may help explain the growth in admissions for asthma and it could be that the NHS has made the eligibility criteria for children having their tonsils removed too tight so that some with recurrent tonsil problems end up needing hospital care, according to Klaber.


He urged paediatricians to work more closely with GPs, health visitors, school nurses and other primary care health professionals in their area so they can help them identify and manage illness in children.


Klaber said a scheme in which children’s doctors from St Mary’s did that with seven “hubs” showed that approach worked. “As well as extremely positive patient and staff experience, our published evaluation of the first full hub showed that 39% of new patient hospital appointments were avoided altogether and a further 42% of appointments were shifted from hospital to GP practice,” he said.


“In addition, there was a 19% decrease in sub-specialty referrals, a 17% reduction in admissions to hospital and a 22% decrease in A&E attenders.”

The study, by researchers at the Health Foundation charity, found that older patients who saw the same GP most of the time were admitted to hospital 12% less for conditions that could be treated in doctors’ surgeries than those who had a lower continuity of care.


The researchers looked at ambulatory care sensitive conditions: these include long-term conditions such as asthma, where flare-ups can be reduced by high-quality GP care, acute conditions such as gangrene that can be prevented by timely treatment, and illnesses such as flu and pneumonia that can be prevented through vaccination.


Admissions for these types of illness are expensive for the NHS: hospital admissions for conditions that could be treated at doctors’ surgeries cost £1.42bn in 2009-10, or more than £170,000 for each GPs’ practice.


Under changes to the GPs’ contract introduced in 2014, all patients in England must have a named GP who is accountable for their treatment. But researchers at the University of Bristol warned in response to the BMJ study’s findings: “There is evidence that continuity of primary care is actually declining in the UK.”


In an editorial for the BMJ, they called for policies to help ensure greater continuity of care.


Seeing the same doctor more of the time could reduce hospital admissions because it “builds trust and a sense of mutual responsibility between patients and GPs”. Older patients were particularly likely to appreciate seeing the same GP, although they were less likely to receive it, the researchers said.


The editorial added: “A primary care system that is increasingly fragmented, in which neither patients nor doctors feel strongly connected to their local general practice, provides the setting for patients to choose to attend an emergency department.”


Difficulty in getting appointments at GP surgeries, “which are at least as overwhelmed as emergency departments”, compound the problem.


Improving the proportion of the time that patients saw the same GPs would improve doctors’ job satisfaction, and was “very likely” to reduce pressure on hospitals, they added.


The Health Foundation researchers analysed patient-level data for more than 230,000 people in England aged between 62 and 82, focusing on older patients because they make up a high proportion of GP appointments and avoidable hospital admissions.


Patients who experienced a medium level of continuity of care had almost 9% fewer avoidable admissions, while those who saw the same GP a high proportion of the time saw just over 12% fewer avoidable hospital admissions in comparison with patients who had a low continuity.


The researchers found a particularly pronounced association between continuity of primary care and reduced hospital admissions among patients who were frequent users of primary care, which they defined as more than 18 visits over the two-year period covered by the data.


The study was observational, and did not prove cause and effect, the researchers emphasised. But improving the continuity of care could reduce hospital costs, especially for the heaviest users of the system, they added.


A spokeswoman for the Department of Health said: “This government is committed to making sure patients can get the right care at the right time from well-resourced GPs and, from April 2015, all patients have had the right to continuity of care through a named GP, as part of the GP contract.


“This remains a key part of our plan to reduce pressure on hospitals, and is in no way diminished by extending access.”

At the wealthy end of the scale, South Cheshire performs badly on inequalities, while East Surrey CCG performs well, appearing in the top 10.


Prof Richard Cookson, of the Centre for Health Economics (CHE) at York, led the team that helped devise the indicators for NHS England. “For the first time, you can now find out how fair your local NHS is, compared with similar local areas, in terms of tackling health inequalities between rich and poor,” he said.


“It’s not simply that Brexit-voting deprived areas do badly, and posh southern suburbs do well. Some deprived areas do well at tackling local health inequalities – such as Tower Hamlets, Stoke-on-Trent, Portsmouth – and some affluent areas do badly – such as South Cheshire and Windsor.” The information is available on a CCG website.


The indicators show how well a CCG is doing in reducing the inequalities that exist between the rich and poor in an area. Although poorer people are likely to have worse health than richer ones, local NHS efforts can reduce this.


The indicators measure the proportion of people from rich and poor backgrounds who have long-term conditions, from dementia to diabetes and heart disease, which are not properly managed at the GP level or in out-patient departments. These are people who end up being admitted to hospital as an emergency because their condition has unnecessarily worsened.


“These emergency admissions are not just a barometer of wider social ills, but also an indicator of how well the NHS is succeeding in delivering out-of-hospital services to deprived patients with complex long-term conditions,” Cookson said. “This is important for NHS managers to know, because social inequalities in potentially avoidable emergency hospitalisation impose large and rising costs on the NHS, as well as raising important concerns about social justice.”


The work was funded by the National Institute of Health Research. Between April 2015 and April 2016, the York team found there were 264,000 preventable hospital admissions in England associated with socioeconomic inequality.


The best performing CCG was City and Hackney in London, followed by Fareham and Gosport, East Surrey, Crawley and the Isle of Wight. Tower Hamlets in London was seventh.


The worst performer was Central Manchester. North Manchester and South Manchester CCGs were also in the bottom five, as were Blackburn,Darwen and Islington.


Miqdad Asaria, the CHE’s lead analyst, said: “There is a social gradient in preventable emergencies meaning everyone, not just the poorest, is affected. The further down the gradient a person is, the greater the chances of suffering a preventable emergency hospitalisation.


“The NHS can now start producing our equity indicators on an up-to-date, annual basis to help improve the coordination of care and reduce preventable hospitalisation and mortality arising from social inequality.”


Ruth Passman, the deputy director for equality and heath inequalities for NHS England, said being able to compare the performance of the NHS around the country on the issue was of major importance.


“For the first time, NHS managers can now find out how well their local CCG area is doing in tackling these inequalities, compared with similar CCG areas, and see how these inequalities are responding to local healthcare initiatives,” she said. “This will help NHS managers learn about the best ways of reducing costly health emergencies associated with social deprivation.”


Islington CCG, which was the fifth most deprived borough in London, said: “There are areas of great and increasing affluence next door to neighbourhoods experiencing some of the most significant deprivation in the capital.”


It said it was working to improve the diagnosis and management of long-term conditions, keeping people in or close to their homes, and had set up programmes to identify people with complex needs who were at risk of ending up in A&E. It had made “significant and sustained progress on reducing inequalities in cardiovascular disease in recent years”, it said in a statement.


A statement from Manchester’s CCGs said: “Manchester has a lot of high-intensity pockets of deprivation. Other ‘norm’ areas, for example Trafford, will have extremes, ie, areas of affluence against areas of deprivation, which is normally shown in findings like this.
“The NHS results are not performance indicators – and the rankings are
based on values. Manchester CCGs see that the tool is useful in highlighting inequality and will see it as an indicator of variables of the current picture – and will continue with their work, especially via the neighbourhood teams, to reduce health inequalities across Manchester.”
