Depression is leading cause of disability worldwide, says WHO study

Cases of depression have ballooned almost 20% in a decade, making the debilitating disorder the leading cause of disability worldwide, the World Health Organization (WHO) has said.


By 2015, the number of people globally living with depression, according to a revised definition, had reached 322m, up 18.4% since 2005, the UN agency said on Thursday.


“These new figures are a wake-up call for all countries to rethink their approaches to mental health and to treat it with the urgency that it deserves,” WHO chief Margaret Chan said.


According to the agency’s definition, depression is a “persistent sadness and a loss of interest in activities that people normally enjoy, accompanied by an inability to carry out daily activities for two weeks or more”.


Lack of energy, shifts in appetite or sleep patterns, substance abuse, anxiety, feelings of worthlessness and thoughts of self-harm or suicide are also common, and can affect entire families.

The drop in productivity, and other medical conditions often linked to depression, also takes a financial toll, with the global cost estimated at $ 1tn annually, the WHO said.


Shekhar Saxena, head of the agency’s mental health and substance abuse department, said both psychosocial and medical treatments could be highly effective, insisting on the importance of reaching more of those in need.


Even in the most developed countries, about half of people suffering from depression are not diagnosed or treated, and the percentage soars to between 80-90% in less developed nations.


Treatment can be difficult to access, while a fear of stigma also prevents many people from seeking the help required to live healthy and productive lives, the agency said.


According to the WHO, every dollar invested in improving access to treatment leads to a return of $ 4 in better health and productivity.


And “early identification and treatment of depression is a very effective means of decreasing death by suicide,” Saxena said.


There are about 800,000 suicides worldwide every year, amounting to one every four seconds. And the link to depression is clear.


Saxena pointed to studies showing that 70-80% of people who kill themselves in high-income countries, and around half of those in low-income countries, suffer from mental disorders, of which depression is the most common.

Cancer drug prices must come down, say leading research institutes

The high price of new cancer drugs is indefensible and unsustainable, say two of the world’s leading cancer research institutions, who propose a different way to develop them that could sideline big pharma.


“There is a clear and urgent necessity to lower cancer drug prices to keep lifesaving drugs available and affordable to patients,” say leading scientists from the Institute of Cancer Research in the UK and the University of Texas MD Anderson Cancer Center, where many important new cancer drugs have been invented, in a paper in the journal Cell.


In the US, cancer bills are the leading cause of personal bankruptcy, while in the UK, drugs that might prolong life are rejected for NHS use because of their price. Many new drugs have to be used in combination, adding to the cost. Treatment with the two new immunotherapy drugs nivolumab and ipilimumab costs $ 252,000, which is more than the median cost of a US home ($ 240,000 in 2016), they write.


Fantastic scientific work is going on – for instance, in sequencing cancer genomes – which should lead to advances in treatment, said Prof Paul Workman, chief executive of the Institute of Cancer Research in London, which is the world’s most successful academic cancer drug discovery organisation. “All this invention is meaningless if patients cannot afford these drugs,” he said.


“It is unsustainable. For those of us involved in research, it is disturbing that the amount of research that goes on and the success that is made is not translated into treatment for patients. And for patients it is a terrible situation.”


Pharmaceutical companies used to justify their prices by pointing to the high cost of clinical trials involving many thousands of patients. But that is no longer always necessary, the scientists say in their paper. The new targeted drugs require a test for a genetic biomarker to see whether patients will respond or not. That means the drug can be trialled on far fewer people. The drug crizotinib, used for advanced lung cancer, was approved following a trial involving only 347 patients, they point out. Trastuzumab (Herceptin) was first approved for advanced breast cancer and later for early breast cancer, increasing the market for the company but with no reduction in price.


“Some drugs are tested on 50 or 100 patients and yet these drugs still go to Nice [the National Institute for Health and Care Excellence, which decides whether the NHS can afford a new drug] at the maximum price,” said Workman.


Workman, together with colleagues from the US and the Netherlands, proposes that academic discovery centres like his should forge relationships with new commercial partners – probably not the major drug companies but smaller biotech or generic drug firms.


Academics should take greater control of the drugs they discover, they argue, and join with small companies that will agree to cap the price when the drug reaches the market. They would not have the expectation of big profit margins, as the major pharmaceutical companies do. But in an era where drugs are tested on smaller populations and genetic testing means they are more likely to be effective, they would not need to “cost in” all the failed attempts at producing blockbusters, as the big companies do.


Workman said the institute was already talking to small companies about the possibility of a new way of developing more affordable cancer drugs. He believes other scientists will support the ideas in the paper. “We’re calling for a more mature and open conversation about how this could be done and offering a solution,” he said.

Dementia and Alzheimer"s leading cause of death in England and Wales

Dementia and Alzheimer’s disease has become the leading cause of death in England and Wales for the first time, figures show.


An ageing population and better diagnosis of the condition has led to dementia and Alzheimer’s knocking ischaemic heart disease from the top spot, statisticians said.


Of the 529,655 deaths registered in England and Wales in 2015, 11.6% were attributable to dementia or Alzheimer’s, according to the Office for National Statistics (ONS).


Among those aged 80 or over, dementia and Alzheimer’s accounted for 13.7% of male deaths and 21.2% of deaths among women.


Meanwhile, the mortality rate for dementia and Alzheimer’s has more than doubled over the past five years, the figures show.


But due to improvements in treatment, diagnosis and awareness, the mortality rates for the other top five leading causes of death – including ischaemic heart disease, chronic lower respiratory disease, lung cancer and cerebrovascular diseases such as strokes – have fallen since 2001.


The ONS data shows that for 2015, ischaemic heart disease was the second leading cause of death, accounting for 11.5% of deaths.


But when the figures are broken down by gender, they show that heart disease is still the leading cause of death for men and dementia and Alzheimer’s disease were certified as the main cause of death among women.


“In 2015, dementia and Alzheimer’s disease became the leading cause of death in part because people are simply living longer but also because of improved detection and diagnosis,” said Elizabeth McLaren, the head of life event statistics at the ONS.


The figures also show large variation among age groups – people over 80 were more likely to die from dementia and Alzheimer’s – while suicide was the leading cause of deaths for people aged under 35.


Meanwhile, breast cancer remains the leading cause of death for women, aged 35 to 49.


But when all forms of cancer are grouped together, cancer was the most common cause of death in 2015, accounting for 27.9% of all deaths.

Many procedures give "no more benefit than doing nothing" – leading doctors

A number of common treatments and procedures routinely undertaken by doctors are largely pointless, leading physicians have said.


The Academy of Medical Royal colleges (AMRC) said that many treatments for conditions from minor grazes to terminal cancers often give no more benefit to the patient than simply doing nothing.


The body, which represents 22 royal medical colleges, said that some broken bones, for example, would not always require a plaster cast and that an x-ray was not always necessary for patients with back pain. The college said it wanted doctors to think carefully about whether certain procedures were required, given their possible side-effects.


“We all have a duty to look after resources in healthcare, especially when the NHS is under so much pressure, but that’s not the main motivation for this initiative,” said professor Dame Sue Bailey, the AMRC chair.


“What’s much more important is that both doctors and patients really question whether the particular treatment is really necessary. Medicine or surgical interventions don’t need to be the only solution offered by a doctor and more certainly doesn’t always mean better.”


The college identified dozens of conditions for which common treatments were not necessarily beneficial, considering the side-effects they can produce.


“Studies have shown that drinking-quality tap water is just as effective for cleaning and washing cuts and grazes as sterile saline solution,” it said. It also said that broken bones in the feet “do not usually need to be put into a plaster cast as they will heal just as quickly in a removable boot”, nor does a child’s wrist when it suffers a certain type of fracture.


And the college said routine cholesterol level checks were not needed for people taking statins who did not have “pre-existing conditions, such as a heart attack, a stroke or a family tendency towards problems with high lipids”.


Other conditions it identified, included terminal cancers – particularly those that that have not responded well to previous courses of chemotherapy. The treatment is “by its very nature toxic”, the college said. “Therefore, the combination of failing to achieve a response and causing toxicity can ‘do more harm than good’.”


The Department of Health declined to comment.

Suicide rate hits 10-year high and dementia poised to become leading cause of death

The number of Australians taking their own lives has hit a 10-year high, while dementia is tipped to overtake heart disease as the nation’s leading cause of death within five years.


The number of suicide deaths climbed above 3000 in 2015 for the first time, rising more than 5% in 12 months, official figures show.


Suicide was the leading cause of death for 15-44 year olds, with males three times more likely than females to take their own lives.


Suicide rates were highest in the Northern Territory, while Queensland recorded the greatest increase in deaths.


The Australian Bureau of Statistics found that while suicide accounted for less than 2% of overall deaths in Australia in 2015, it claimed the lives of a third of those aged 15-24 and more than a quarter of 25-34 year olds.


Mental health groups were dismayed by the rising rates and called for a new national approach to suicide prevention.


The chairman of beyondblue, Jeff Kennett, said the Senate should set up a special commission to combat suicide, while Lifeline wants a national summit.


“These figures have to stop us in our tracks,” Kennett said. “The 2015 total is two-and-a-half times the national road toll and six times the number of Australian lives lost in the entire Vietnam War.”


The chief executive of Sane Australia, Jack Heath, said while much had been done to reduce the stigma around mild and moderate mental health issues, more help was needed among the 700,000 Australians with more complex conditions.


“There needs to be access to better-quality services and that particularly becomes an issue for people in rural and regional areas where there are a quarter of the psychiatrists and half the psychologists,” he said.


“And we need to do a better job to help people who are discharged from emergency departments after suicide attempts by making sure there’s better follow-up services.”


While deaths from suicide are rising, heart disease remains the main cause of death for most Australians with more than 19,700 fatalities last year.


However the ABS predicts dementia will topple heart disease from its top spot by 2021, with the number of deaths having doubled in the past decade to 12,625 largely as a result of our ageing population.


Dementia is now the second-biggest killer of Australians, while heart disease and stroke-related deaths have been steadily declining.


Alzheimer’s Australia’s CEO, Maree McCabe, said while one in 13 people in their 30s, 40s and 50s have the brain disorder there was not enough awareness about dementia among young and middle-aged people, who often mistakenly think dementia is a normal part of ageing.


“If we can delay the onset of dementia by five years we would reduce the number of people who get dementia by 30%,” she said.


McCabe called for a government-funded national dementia strategy, similar to those in the US and Britain.


She also advises people reduce their risk of developing dementia by exercising, eating healthily, socialising and keeping their brains active.

Leading ocean advocacy groups join forces to tackle microfiber pollution

Two leading clean ocean advocacy groups have joined forces to stop tiny synthetic clothing fibers from polluting the world’s waterways and poisoning the food chain.


Plastic Soup Foundation (PSF), a Dutch nonprofit, and New York-based Parley for the Oceans announced Tuesday a partnership to tackle the issue of microfiber pollution and to create a global alliance of companies, governments, NGOs and scientists. Microfibers – tiny, often synthetic threads shed from laundry, industrial clothing manufacturing and fishing nets – have been found in alarming numbers in recent studies of microplastic pollution.


“Microfibers pose a massive and complex threat,” said Cyrill Gutsch, founder of Parley for the Oceans, in a statement. “It’s all about funding, supporting and boosting new technologies… [and] the best way to achieve this is together.”


The partnership comes after PSF launched its Ocean Clean Wash campaign earlier this year to educate washing machine manufacturers, fashion brands, environmental organizations and the public about the problem. PSF says already 100 nonprofits and other organizations have signed on to support the campaign, including Greenpeace UK and Austria, Plastic Pollution Coalition and Fauna & Flora International.


A June study found that synthetic fleece jackets release, on average, 1.7 grams of microfibers each wash. These small fibers aren’t easily filtered out at local wastewater treatment plants. The study, funded by outdoor clothing company Patagonia, found that up to 40% of these small fibers land up in oceans, rivers and lakes, threatening wildlife and ultimately our food chain.


Synthetic fibers such as acrylic, polyester and nylon pose the most danger because they don’t disintegrate and disappear over time. Instead, they break down into smaller, increasingly toxic pieces, allowing them to be consumed by fish and other animals. These tiny fibers have the potential to build up and lead to a potentially poisonous accumulation of chemicals in larger species. A study from the University of Exeter found that crabs contaminated with microfibers ate less food and that the polypropylene plastic broke into smaller pieces when ingested, leading to a greater spread in the body – and consequently a greater threat to human health.


Clothing companies have been slow to lend their support to the issue, although that’s starting to change. Patagonia says it continues to fund research and provide grants to explore ways to reduce the release of microfibers into waterways. Dutch clothing brand G-Star Raw announced earlier this year it was supporting PSF to raise awareness among fashion and textile companies about the damage their clothing can pose to the environment and to public health. According to PSF, nearly 40 fashion brands have agreed to collaborate as part of the partnership with Parley. The brands involved will be announced during a Call to Action event early next year.


PSF and Parley will work with other organizations to launch a competition next year to encourage scientists, inventors and clothing makers to come up with ways to stop the release of microfibers in the wash. The winners will be announced in 2018, and will receive a cash prize to implement their ideas.


According to PSF and Parley, possible solutions to the issue include a mechanical filter in washing machines to catch tiny fibers, synthetic yarns and fabrics that don’t release fibers in the wash and environmentally safe coatings that prevent microfibers from releasing when rinsed.

People living in affluent areas of England are just as likely to end up in hospital because of inadequate healthcare as those living in impoverished areas, statistics have shown.


Data compiled by the University of York for NHS England revealed that the wealth or poverty of an area makes little difference to how well the local NHS treats its patients.


Liverpool clinical commissioning group, which appears on the worst performers list, and Tower Hamlets and Portsmouth CCGs, which appear on the best performers list, each serve some of the most deprived neighbourhoods in the country.


At the wealthy end of the scale, South Cheshire performs badly on inequalities, while East Surrey CCG performs well, appearing in the top 10.


Prof Richard Cookson, of the Centre for Health Economics (CHE) at York, led the team that helped devise the indicators for NHS England. “For the first time, you can now find out how fair your local NHS is, compared with similar local areas, in terms of tackling health inequalities between rich and poor,” he said.


“It’s not simply that Brexit-voting deprived areas do badly, and posh southern suburbs do well. Some deprived areas do well at tackling local health inequalities – such as Tower Hamlets, Stoke-on-Trent, Portsmouth – and some affluent areas do badly – such as South Cheshire and Windsor.” The information is available on a CCG website.


The indicators show how well a CCG is doing in reducing the inequalities that exist between the rich and poor in an area. Although poorer people are likely to have worse health than richer ones, local NHS efforts can reduce this.


The indicators measure the proportion of people from rich and poor backgrounds who have long-term conditions, from dementia to diabetes and heart disease, which are not properly managed at the GP level or in out-patient departments. These are people who end up being admitted to hospital as an emergency because their condition has unnecessarily worsened.


“These emergency admissions are not just a barometer of wider social ills, but also an indicator of how well the NHS is succeeding in delivering out-of-hospital services to deprived patients with complex long-term conditions,” Cookson said. “This is important for NHS managers to know, because social inequalities in potentially avoidable emergency hospitalisation impose large and rising costs on the NHS, as well as raising important concerns about social justice.”


The work was funded by the National Institute of Health Research. Between April 2015 and April 2016, the York team found there were 264,000 preventable hospital admissions in England associated with socioeconomic inequality.


The best performing CCG was City and Hackney in London, followed by Fareham and Gosport, East Surrey, Crawley and the Isle of Wight. Tower Hamlets in London was seventh.


The worst performer was Central Manchester. North Manchester and South Manchester CCGs were also in the bottom five, as were Blackburn,Darwen and Islington.


Miqdad Asaria, the CHE’s lead analyst, said: “There is a social gradient in preventable emergencies meaning everyone, not just the poorest, is affected. The further down the gradient a person is, the greater the chances of suffering a preventable emergency hospitalisation.


“The NHS can now start producing our equity indicators on an up-to-date, annual basis to help improve the coordination of care and reduce preventable hospitalisation and mortality arising from social inequality.”


Ruth Passman, the deputy director for equality and heath inequalities for NHS England, said being able to compare the performance of the NHS around the country on the issue was of major importance.


“For the first time, NHS managers can now find out how well their local CCG area is doing in tackling these inequalities, compared with similar CCG areas, and see how these inequalities are responding to local healthcare initiatives,” she said. “This will help NHS managers learn about the best ways of reducing costly health emergencies associated with social deprivation.”


Islington CCG, which was the fifth most deprived borough in London, said: “There are areas of great and increasing affluence next door to neighbourhoods experiencing some of the most significant deprivation in the capital.”


It said it was working to improve the diagnosis and management of long-term conditions, keeping people in or close to their homes, and had set up programmes to identify people with complex needs who were at risk of ending up in A&E. It had made “significant and sustained progress on reducing inequalities in cardiovascular disease in recent years”, it said in a statement.


A statement from Manchester’s CCGs said: “Manchester has a lot of high-intensity pockets of deprivation. Other ‘norm’ areas, for example Trafford, will have extremes, ie, areas of affluence against areas of deprivation, which is normally shown in findings like this.
“The NHS results are not performance indicators – and the rankings are
based on values. Manchester CCGs see that the tool is useful in highlighting inequality and will see it as an indicator of variables of the current picture – and will continue with their work, especially via the neighbourhood teams, to reduce health inequalities across Manchester.”
