I"ve had to remove all of a toddler"s teeth. It"s time for a war on sugar

It is the end of an afternoon in theatre and I have extracted more than 100 teeth from my operating list of eight patients, the youngest a two-year-old who needed all 20 baby teeth removing because they were so decayed. I watch in silence as a child younger than my own is transferred from the operating table and I wonder how we reached this point as a society where I don’t believe we truly value oral health, nor realise the implications of failing to do so.


Nearly 20 years after observing my first general anaesthetic as a student it doesn’t get any easier. I regularly see parents overcome by guilt and emotion as they watch their child being put to sleep, or recovering dazed and confused in the recovery suite. Sometimes after a busy afternoon I sit in the theatre and wonder if there is more I can do, sometimes I have nothing left to give.




Responsibility for oral health promotion has been devolved to cash-strapped local authorities




As an NHS consultant in paediatric dentistry, it sadly comes as no surprise to me that removal of decayed teeth remains the most common reason for a child aged five to nine years to be admitted to a hospital in England. In these straitened times, it seems so wrong that every year we spend around £35m on operations to treat a disease that is almost always preventable.


The frustration is that the solutions are already out there. Ten years ago, the Scottish government agreed to invest in a programme of oral health prevention called Childsmile. Now every child in Scotland has access to free daily supervised toothbrushing in nursery and free dental packs to support toothbrushing at home. Dental registration is encouraged and those communities and individuals who are higher risk have more support. The result? Scotland is reducing the millions of pounds it spends on general anaesthetics and turning around the oral health of its children, for the princely sum of £17 per child per year.


Wales has a similar Designed to Smile programme but in England, responsibility for oral health promotion has been devolved to cash-strapped local authorities. This means that it is a postcode lottery with some excellent programmes, such as Teeth Team in Hull, while in other areas existing services are being decommissioned.


England needs urgent investment in oral health prevention. It is actually more cost effective to prevent, rather than treat, dental disease but more importantly we could be preventing tens of thousands of young children, and their families, from potentially experiencing pain, swelling and sleepless nights and time away from school or work.


Every child has a right to good oral health yet still we see one in eight three-year-old children with obvious signs of decay. We need a more radical approach to reduce the persistent inequalities in oral health, which are immoral in this day and age. We need more compassion, an accelerated programme of product reformation so that the sugar content is reduced, and a war on marketing of high sugar products aimed at children.


Two years ago after a particularly frustrating consultation with a parent whose child’s diet was limited to Ribena and biscuits, I decided to begin writing a blog as a way of delivering practical information. I have reached many more families than I would in my day job, but I still feel as though I am wading in a sea of untreated decay. Education is important but it forms only part of the solution. It is too simplistic, and frankly unhelpful, to apportion all blame to the parents. Yes, as parents we have responsibilities and a vital role to play – but we could all be more proactive when it comes to children’s oral health.


I am delighted at the growing number of organisations that are now beginning to collaborate with myself and colleagues via the British Society of Paediatric Dentistry. Health visitors can and are encouraging a dental check before a baby’s first birthday; sports and education settings can lead by example, adopting low-sugar menus and refusing to place sugary drinks in their vending machines; and communities should campaign for water fluoridation. Above all, we need strong, visible leadership by local and national government that says, “This is not acceptable and we are going to do something about it”. Children’s oral health should be everyone’s business.

3 Ways To Naturally Clean Your Teeth

A lot of people seem to neglect the amazing properties that most natural products have when it comes to treating a variety of health issues – whether in their organic forms or when processed into a natural product or supplement. I think one surprising area where natural products are highly effective, is oral hygiene.  – And it’s one of the most important factors in keeping your body healthy and well regulated. As a matter of fact, recent studies have shown that roughly 80-85% of bacteria that can cause or lead to heart disease are found in our mouths and the plague that forms there! So it’s important to keep your mouth clean for more reasons than simply having a nice smile or fresh breath.


Like most of us, I’ve used normal fluoride based toothpaste for most of my life – largely because I never really knew there were any alternatives and secondly because I never thought there was anything wrong with them – even though one of the most obvious downsides of using fluoride toothpaste happened to me after every rinse. I’m referring to that dry, cotton-like feeling that happens to your mouth after you rinse when using the paste. It’s really uncomfortable and can actually worsen your breath – rather ironically.


As I grew up, I used to treat this with antibacterial mouthwash which worked a bit, but still left me with a dry mouth and sensitive teeth afterwards. There are other toothpastes or mouth products that are designed to help tooth sensitivity, but these cost a small fortune and still leave you with a dry mouth. It was at this point that I started researching natural methods of keeping your teeth clean and what I found really surprised me. There are so much cheaper, non-toxic and purely organic methods out there, that I still don’t really know why most of us are using standard mouth care products. So, in an attempt to open your eyes to countless healthy and cost-saving options out there, I’ve listed a 3 of them below – and hope you’ll work for you as they have for me.


Oil Pulling


This is one of the oldest oral hygiene techniques in human history and has been used in Ayurvedic healing practices for more that 3000 years! Anything that’s managed to stand the test of time for that long must have some element of truth behind it – but after trying it myself, and I can definitely confirm that it works. It’s real simple too!


Basically, oil pulling is just swishing an oil around in your mouth for 15-20 minutes and then rinsing it out. It works as a natural antibacterial mouthwash and helps to extract any germs or bacteria that may be lingering in your gums or cavities, as well as ridding you of bad breath and even helping to treat acne by stimulating cell regeneration in the skin layer around your mouth.


I recommend using Coconut, Canola, Sesame or Olive oil for this process. You can even mix a few drops of essential oil into the oil to strengthen its healing properties and help ward off any bacteria. Oils such as peppermint, tea tree or eucalyptus work like a charm in your mouth and will also give you fresher smelling breath.


Natural Toothpaste


A great way to get the same effect on your teeth as you do with normal toothpaste is to switch your fluoride based option, with a homemade DIY recipe that is as effective as the shelf-bought stuff, it’s just a lot healthier and won’t dry your mouth out either!


To do this, all you will need is:


– ½ cup of Coconut Oil
– 3 tablespoons of Baking Powder or 4 tablespoons of Xylitol
– 20 drops of Peppermint Essential Oil
– 10 drops of Eucalyptus or Tea Tree Oil
-A small glass jar or sturdy plastic container

Making the toothpaste couldn’t be easier! All you have to do is melt the coconut oil in a pot or pan. Once the oil is in liquid form, add all the other ingredients starting with the baking powder/xylitol and then pouring in the essential oils. Make sure you keep stirring the liquid so that all the ingredients dissolve into the mixture. Now, take the pot or pan off the heat and allow the mixture to cool. As it begins to harden, decant the mixture into the the glass or plastic container and store in a cool, dry cupboard out of direct sunlight.


To use, simply dip your toothbrush into the jar and spread a little on the bristles. Then simply brush normally and rinse afterwards. And voila! DIY natural toothpaste alternative!


Natural Strawberry Teeth Whitener


Over the years, the amount of tannins that our teeth come into contact with through the food we eat, wine, tea and coffee that we eat is immense! What are tannins you might ask? These are naturally occurring compounds (polyphenols) that add colour to certain foods as well as the rinds of certain fruits. They generally occur in food produced from plants and are strong natural antioxidants – so are really great for flushing out any toxins in your bloodstream.


However, the one downside to tannins is that they cause discolouration in our teeth and often cause them to go yellow over time. Some people turn to really expensive teeth whitening treatments that generally make your teeth look unnaturally white and also involve the use of some really nasty chemicals.


I recommend using a strawberry teeth whitener on  daily basis as a way of prolonging tooth discolouration and saving you a tonne of cash on costly teeth whitening treatments. It’s literally as easy as crushing up about 4-5 strawberries and rubbing the paste on your teeth. Then simply rinse the paste off and you’ll start seeing the results quickly. I recommend doing this after brushing with your natural toothpaste and oil pulling in the morning (so just before you go to bed). Plus you’ll always have strawberries around the house which is a huge bonus!

Ultimately, oral hygiene is one of the most important aspects of leading a healthy life – anyone who’s ever had toothache or a serious abscess will tell you that. Yet leading a healthy life shouldn’t mean forking out heaps of cash for treatments that make you feel uncomfortable or are made up of ingredients that you can hardly pronounce, let alone understand. As with most things in life, there’s generally a healthier, natural alternative that can save you money and keep you healthier for longer – all you have to do is make the switch. I’ve been using natural oral hygiene products for some time now and have only had success.


So I always say why not give them a chance? They cost less and can only do good – worst case scenario is that they don’t work as well as you’d like them to and you go back to what you were doing, but if you follow the recipes correctly you should have nothing but a clean smile, fresh breath and more cash in your wallet at the end of each month.

Swish the oil around your mouth for about a minute and a half before brushing with plain water or another healthy alternative like baking soda.


5. Strawberries and Tomatoes


Using fruit as a natural substitute is an effective solution in various aspects of life, generally for diets. But they can also be beneficial for your oral health, as well. Both strawberries and tomatoes have high levels of Vitamin C. The Vitamin C will stimulate your body’s natural healing. Child "sugar scourge": thousands having teeth removed in hospital

More than 40,000 children and young people a year are having rotten teeth removed in hospital in further evidence of what doctors call the “costly scourge of sugar”.


New NHS figures obtained by the Local Government Association (LGA) show that 40,800 under-18s in England had at least one tooth taken out last year under general anaesthetic because of decay. Performing the procedures cost £35.6m.


The children involved had such advanced decay that they could not be treated by a dentist and had to go to hospital instead. NHS surgeons carried out 161 such operations each working day.


“These figures are a stark reminder of the damage excessive sugar consumption is doing to our children’s teeth,” said councillor Izzi Seccombe, chair of the LGA’s community wellbeing board.


“It is deeply worrying that the type of dental treatment required is beyond the capacity of a local dentist, due to the severity of the tooth decay, and as a result has to be done in hospital,” she added.


The 161 operations a day should serve as “a wake-up call” to ministers to introduce tougher curbs on sugar, not just the planned levy on sugary soft drinks, said Prof Nigel Hunt, the dean of the faculty of dental surgery at the Royal College of Surgeons.


“The awful impact sugar is having on our children’s teeth is brutally evident in the number of under-18s we are seeing daily that need more than one tooth extracted due to tooth decay. It is shameful that a problem which is 90% preventable continues to plague our children in this way,” Hunt said.


“Having teeth removed can be very traumatising for children and requires time away from school for them, as well as time away from work for their parents.


“The government’s sugar tax will go some way to cutting sugar consumption that leads to tooth decay, but it also needs to support public health campaigns that remind parents of good oral health,” Hunt added.


The LGA, which represents councils in England and Wales, wants ministers to let it help decide how proceeds from the sugar tax should be spent.

Is flossing your teeth a waste of time?

“I don’t need to floss” is a sentence my dentist never used to hear – not until recently, when the latest Dietary Guidelines for Americans stopped recommending it. Now, lots of his patients are telling him they’ve stopped. Despite the NHS advising daily flossing – and almost all dentists being fans of interdental cleaning – an investigation by Associated Press found “little proof that flossing works”.


But how can it not be good for you? Between our teeth are interdental sites coated in dental plaque. Plaque is a layer of bacteria mixed with organic matter that coats teeth and causes gum inflammation (gingivitis) and tooth decay. Early warning signs include bleeding gums. Toothbrushes get rid of plaque from other surfaces, but are not so good between teeth. Surely floss can reach the parts that brushes can’t?


The solution


The lack of good research emerged in a 2011 systematic review by the Cochrane Oral Health Group. Its summary of results from 12 studies found only weak evidence that flossing (plus brushing) reduces bleeding from gums, and none for plaque reduction. But good studies are hard to do. The quality of flossing varies. One study showed that professional flossing five times a week can reduce tooth decay – a result not seen in any studies of self-flossing – so it’s clearly a skill. Experts advise moving the floss, held in a C shape, away from the gum in an up-and-down motion.


However, the Cochrane paper actually said that, although the research was poor, the benefits of flossing probably outweighed any risks. Bleeding gums is a risk factor in developing chronic gum disease than can destroy the bone underneath. Professor Damien Walmsley, a scientific adviser for the British Dental Association, says that brushing teeth twice a day with a fluoride toothpaste and only occasionally eating sugar treats after meals is the best way to prevent tooth decay. He believes flossing is of little value and that small interdental brushes are better at cleaning the gaps between teeth.


No one, however, is arguing that flossing isn’t useful for dislodging unsightly food stuck between teeth. My own dentist, Richard Travers, thinks we can drop the daily enslavement and reduce flossing to twice a week, increasing if our gums start bleeding. His gut feeling, from what he sees, is that both flossing and interdental brushes are useful. And, in a recent feature in the British Dental Journal, senior dentists including Phil Ower, the president of the British Society of Periodontology, announced that they still floss their own teeth.
