Fury as last London community dental clinic shuts

The NHS has been accused of letting down patients in pain by shutting London’s last community-based A&E-style service for people needing emergency dental treatment.


NHS England’s closure of the “urgent dental service” in Kentish Town will leave the capital’s 8.7 million residents with only two busy hospitals to go to with teeth problems. Dentists have warned that it will force more patients to seek help at overcrowded GP surgeries and A&E units. The disappearance is the latest loss of walk-in services across England for people with emergencies such as a broken tooth or abscess.


NHS England said the service would shut on 31 March, even though 5,451 people used it last year and patients come from all over London to get treatment. People should in future call the NHS111 helpline or seek an appointment with their regular dentist, they said.


“Access to emergency dental care is increasingly a postcode lottery. Inadequate provision is simply piling more pressure on GPs and A&Es that are not equipped to provide dental treatment”, said Henrik Overgaard-Nielsen of the British Dental Association.


“It’s absurd that NHS111 operators are asking patients to do ring-rounds [of dental surgeries looking for a free slot at short notice],” he said.


The association is urging the NHS to tackle the growing shortage of emergency dental services by arranging for high-street dentists to always have a certain number of emergency appointments available.


“It is irresponsible to shut a fantastic service that provides emergency dental care to patients without providing an alternative. Where do they think these patients are going to go?” said Dr Francesca Silman, a doctor whose practice is near the closing walk-in centre. “It is not acceptable to leave people in pain and in need of treatment, and this will only worsen the pressures already on GPs and A&E who can offer very little help to those that attend.”


Andrew Dismore, a Labour member of the London Assembly, has voiced concern about the service going, NHS England’s lack of public consultation, and the fact that it has been taken “against the wishes” of Whittington Health, the NHS trust that staffs it.


The dental association recently estimated that 135,000 dental patients a year end up attending A&E because they cannot easily gain access to care for a sudden problem. A further 600,000 seek treatment from a GP, adding to the pressure on family doctor services, it believes.

From April, Londoners needing urgent dental care will have only two options: specialist clinics at King’s College Hospital and Guy’s Hospital, both in south London, though the Guy’s service limits treatment to 55 patients a day and those needing care must be there as early as 7.30am to secure an appointment. The Royal London Hospital in east London recently shut its clinic.


An NHS England spokeswoman said: “We recently reviewed the service and found that there is sufficient capacity for patients who need urgent dental care at existing dental practices and so the closure of the service would not negatively impact patients in the area.


“There are more than 1,250 high-street dentists across London where patients can access dental care when they need it. Londoners in need of urgent dental care should call NHS111, who can then signpost them to the most appropriate service for their treatment.”

The media furore over dental checks to establish the ages of child migrants arriving from Calais has raised the question of how we define – and prove – adulthood. Dentists have said the checks would likely be inaccurate, not to mention unethical. Brain imaging wouldn’t be much use either, as recent research has shown the brain continues developing right up until the mid-20s and beyond.


Growth of grey matter is very rapid from around the age of 12, but peaks a few years later. What follows in later adolescence is a pruning and sculpting of the many connections that have sprouted during puberty. It could be argued that experiences in this period are as important as earlier ones, and in some ways more significant, as they affect the brain’s frontal lobe which deals with the development of character and morality.


Insurance companies seem to have realised this, as car insurance premiums change depending on your age right into your 20s. And since 2013, child psychologists have worked with under 25s, not just under 18s. It’s just a shame that this is not currently reflected by our asylum policy.

What I’m really thinking: the dental hygienist

You may think you’re coming in “just for a clean” but it’s my job to check your oral cavity for lesions, your teeth for caries and your gums for periodontal disease, and to provide advice to get you to full oral health. Many a joke is made about relaxing in the chair. I call it the comfy chair; it breaks the tension. I know it’s not the most pleasant experience, so I try to alleviate the worry by explaining the emotions people feel as they lie back. After all, I am “exposing” your vulnerable, soft places – your belly and chest – and instinct prompts the nervous patient to cross their arms. Then, as I start the treatment, I am right there in your personal space, wearing a mask and glasses – another uncomfortable feeling for you.


I like my job and people mostly leave feeling better, wiser and – yes – cleaner. I don’t mind how bad your teeth are, it’s bloody satisfying getting a load of tartar off, leaving smooth enamel to rub your tongue around instead of rough rock. But I have feelings too; I can’t help looking at your blackheads and up your nose at your nasal hair (or worse). You can’t blame me: I have excellent lighting.


I find it rather creepy when your tongue follows my instruments and irritating when it nudges them out of the way, and when you look me in the eye as I move into your vision. And why, all of a sudden, can’t you swallow your own saliva? But the worst thing is the disrespect some people show by not cleaning their teeth or by eating just before seeing me. I’d never say it, but I so want to ask: “Would you leave another bodily cavity in the same condition if you were visiting your gynaecologist for a smear test or a urologist for a prostate exam?” I don’t think so.

Why are people taking dental complaints to their GP?

Why are so many people going to see a GP rather than their dentist when they have problems with their teeth?


About 600,000 people a year in the UK who develop dental problems now visit a GP rather than going to a dentist, the British Dental Association revealed last week. That is roughly 11,000 people a week. The BDA, which represents dentists and dental students, blames the rising cost of dental treatment. It sees charges for dental treatment as “a tax on health”. It is also worried that such cases are adding to the growing pressures on already overstretched doctors and costing the NHS an estimated £26m.


In the last dental health survey, 26% of adults said the cost involved had been a factor in choosing the type of dental treatment they received, while 19% said they had delayed having treatment for the same reason.


Is this a new problem, perhaps caused by the NHS-wide financial squeeze?


Not exactly. In 2013, a Comres survey of 1,007 GPs across the UK found that “87% of GPs feel under increasing pressure from patients who are coming to them, rather than their dentists, for oral health-related issues”.


In addition, 96% of GPs want more to be done to discourage patients with dental problems seeking their help, Comres found.


Can GPs do anything to help?


Not really. People come with tooth pain, abscesses and other ailments. However, “GPs are neither trained nor equipped to deal with these problems, particularly those that require an operative intervention. GPs can offer pain relief or antibiotics, but will usually refer patients to a dentist”, says Henrik Overgaard-Nielsen, chair of the BDA’s general dental practice. “The overwhelming majority of these patients require root treatment or extraction, which GPs cannot provide. GPs can treat symptoms, not causes. Painkillers and antibiotics are never a solution to dental problems.”


For example, a patient with an abscess will often need the infection to be drained or the tooth taken out; GPs can do neither procedure.


Dr Maureen Baker, chair of the Royal College of GPs, agrees. “GPs are not dentists. Patients who are experiencing pain or discomfort with their teeth and/or gums should seek an appointment with their local dentist, not their family doctor. GPs are in no position, and shouldn’t be put in a position, to treat patients with dental problems,” she says.


How much are dental charges? Have they always existed?


Dental care was free when the NHS was created. However, that didn’t last long. In 1951, Clement Attlee’s Labour government, which had set up the service, brought in charges for dental treatment and glasses. That prompted Nye Bevan, who as the health minister had launched the NHS three years earlier, to resign because he felt that its key founding principle – that medical help should be free at the point of need – had been betrayed. In contrast, visits to GPs have always been free.


The BDA is worried that dental charges in England went up by 5% in April and are set to rise by another 5% next year. It claims the increases are designed to force patients to make up for diminishing amounts of government funding for dentistry – down £170m in England since 2010-11, from £2.2bn to £2.03bn.


Charges in England are much higher than in the rest of the UK. It costs £19.70 for an examination there, but only £13.50 in Wales, while such checks start at £6.68 in Northern Ireland and are free in Scotland. However, that £19.70 also includes diagnosis and X-rays or a scale and polish, if you need that.


One leading dentist refutes that this is too costly. “Twenty pounds for an appointment to sort out toothache? You need to decide where this sits in affordability.” But charges can go up quickly if you need fillings, root canal treatment or tooth removal (£53.90) or crowns, dentures or bridges (£233.70).


Many patients on low to moderate incomes get no exemption from dental charges.


How much do dentists earn?


Dentists’ average gross earnings in England and Wales were £155,100 a year in 2013-14. But that figure has been falling in recent years. After deducting their expenses, such as running their premises and buying equipment and supplies, their taxable income is about half that impressive-sounding sum – £74,400 a year. “I’m not saying we don’t make a reasonable living. But it’s no more than the average professional would be making,” the senior dentist says.


“Dentists’ incomes have fallen by 25% in the past decade,” says Overgaard-Nielsen. Plus, surgery running costs are mounting, especially owing to difficulties in recruiting dental nurses.


Would the nation’s teeth be in better shape if all dentistry was NHS-funded?


Dentists complain that NHS dentistry is a “Cinderella service”, its low status confirmed by the gradual scaling-back of government funding. Maintaining charges that deter at least some people on lower incomes is “feeding a false economy”, says Overgaard-Nielsen. “It’s cheaper for dentists to keep healthy teeth in healthy mouths than for GPs to offer pain relief or for hospital surgeons to perform extractions under general anaesthetic.”
