If basic healthcare is a privilege, what rights do we have? | Jamie Peck

A right-wing hero was born this past Sunday during the combination bikini contest and civics test that is the Miss USA pageant. In an effort to probe how she’d solve our nation’s problems, host Julianne Hough asked Miss Washington DC (aka Kara McCullough) if she believed “affordable healthcare for all US citizens” was “a right or a privilege,” and why. McCullough answered by saying:


“I’m definitely going to say it’s a privilege. As a government employee, I’m granted healthcare and I see firsthand that for one to have healthcare, you need to have jobs. So therefore, we need to continue to cultivate this environment that we’re given the opportunity to have healthcare as well as jobs to all American citizens worldwide.”


Currently unemployed people, as well as those with bad jobs that don’t provide healthcare, are presumably out of luck. That McCullough wants to help women find work in Stem jobs is admirable, but does nothing to help the vast majority of people.

In which case, she’s as wrong and out of touch as any DC royalty currently making policy on this issue.


Now, I’ll admit it’s tough to prove beyond the shadow of a doubt that people deserve basic levels of physical safety simply by virtue of being human. To do so would require a philosophical deep dive above my level of expertise. But our founding documents — which I’d hope someone with the surname “USA” respects — have already sided with the “yeas” on the existence of certain inalienable rights, chief among them “life.” And an estimated 45,000 Americans die each year from lack of health insurance. This is not just another commodity, but a necessity on the level with food and shelter. Which, to be fair, Republicans also want to take away from the poor.


Beyond that, it comes down to a simple matter of preference: do you want to live in a society that codifies some level of responsibility to our fellow citizens, or would you rather roll the dice on a Randian dystopia where the lazy, unlucky or otherwise uncompetitive are liquefied into paleo shakes for rich people? Maybe I can’t objectively prove which is “better,” but I know which I’d prefer. Contrary to what social Darwinists would tell you, one cool thing about being human is we (theoretically) get to decide which ideals we want to pursue.


When healthcare is a for-profit enterprise, costs skyrocket and access plummets. While the Affordable Care Act reined in some of the industry’s worst abuses, an estimated 27 million are left uninsured under it, a number that’s slowly climbing as companies raise premiums and pull out of exchanges.


Even those who are insured report problems paying for care due to high deductibles, and, more generally, to being insured by companies whose business model is to dole out as little coverage as they can legally get away with. The Republican party is currently in the process of turning 27 million into 52 million with their ironically named “American Healthcare Act.” Soon, 45,000 deaths a year will seem like the good old days.


As every other country in the developed world knows, the only way to increase coverage to 100% — as well rein in absurdly high per capita spending on a bloated private industry — is with a system of state-funded healthcare. It works everywhere else, and many experts say it would work here.


The American people are on board. A 2017 Economist/YouGov poll found about 80% of Democrats and 40% of Republicans — yes, Republicans! — favor a federally funded health insurance system that covers all Americans. All race, age, income, and gender demographics favor it by simple majorities, and many — particularly the most marginalized — by strong ones. Even Republicans and conservatives approach 50%. Those who voted for Hillary Clinton favor it at a rate of some 85%.


Unfortunately, this is one of many areas where the country’s political class refuses to even remotely entertain the will of the people. Neither Democrats nor Republicans are willing to do the empathetic, fiscally responsible thing.


We all know Republicans will happily let people die to pay for oligarchs’ tax breaks. But even most elected Democrats remain irrationally committed to compromising with an industry that wants you dead. This becomes more comprehensible when you look at the amount of money the insurance lobby gives to both parties. Despite its immense popularity with voters, single-payer will be on neither party’s agenda in 2018.


In stating that affordable healthcare is a privilege that should be reserved for gainfully employed people, McCullough showed she’ll fit in just fine with the other undemocratically elected rulers in DC. President Trump might not own the Miss USA pageant anymore, but his cutthroat spirit lives on.

Millions of UK asthma sufferers "not receiving basic levels of care"

Millions of asthma patients are not receiving basic levels of care to keep their condition in check, a charity has warned.


Two-thirds of sufferers are not being given fundamental care to manage their condition, Asthma UK said. This is about 3.6 million people across the UK.


Full provision of this basic care would “reduce the impact of asthma, reduce hospital admissions and improve the lives of hundreds of thousands of people”, according to a new report by the charity.


Basic care includes having an appropriate asthma review at least once a year – or more often for severe cases and children – being on the right medication and knowing how to use it, and having a written asthma action plan.


The annual asthma survey, based on responses from 4,650 patients from around the UK, found a variation in the proportion of people receiving basic care across the country, with some areas “lagging behind others”.


Patients in Northern Ireland appeared to get the best basic care, with 47.6% receiving all elements of recommended care.


The lowest proportion was in London, where just 27.5% of patients received this level of care.


The report’s authors said it was not just this basic care that was “failing people with asthma”. The report stated that seven out of 10 patients who required hospital care or out-of-hours treatment did not have a follow-up appointment.


The authors said: “Correct discharge arrangements after a hospital stay for asthma saves lives, and this is a particularly worrying finding.”


There are about 5.4 million people with asthma in the UK. In 2015, 1,468 people died from the condition – the highest number for more than a decade. The charity said two-thirds of asthma deaths were preventable with the right basic care.


“It is worrying that basic care is not being delivered on a consistent basis because every person with asthma should be receiving this care,” said the charity’s clinical lead, Dr Andy Whittamore.


Kay Boycott, chief executive of Asthma UK, said: “With the 2014 national review of asthma deaths reporting two out of three asthma deaths are preventable with good basic care, it is hugely disappointing that the latest Asthma UK care survey shows little has changed since that damning report.


“It is clear that expecting old ways to tackle long-standing problems won’t work. We must take a bold new approach and take advantage of new asthma digital health solutions to transform the way asthma care is delivered and support self-management.


“Digital asthma action plans, smart inhalers and automated GP alerts are just some of the ways asthma care could be brought up to date and help reduce the risk of potentially fatal asthma attacks.”

GPs are failing to carry out basic checks on children with fever that could flag up life-threatening illnesses, research suggests.


A study found that most under-fives with a high temperature were not checked properly for signs that could indicate sepsis, meningitis or pneumonia.


The National Institute for Health and Care Excellence (Nice) published guidelines in 2007 and again in 2013 urging GPs to carry out four simple tests. But evidence suggests they are still not being done and charities say lives are being needlessly lost as a result.


The Nice guidance says GPs must measure and record temperature, heart rate, respiratory rate and capillary refill time as part of the routine assessment of a child with fever.


Capillary refill is when doctors press on a patient’s nail bed and then count the seconds until it returns to its usual colour – normally under two seconds.


In the study, Dr Alice Lee, from Pennine Acute Hospitals NHS Trust, looked at data for 47 under-fives in a Stockport GP practice. She found just 13% of consultations included all four checks, with about one in three children receiving no tests at all or only one.


Despite the fact abnormal respiratory rate was often the first sign of serious illness, Lee said it was the lowest-recorded test, with only 30% of children being checked.


Almost half of youngsters also missed out on having their pulse taken, while a quarter did not have their temperature recorded.


But when GPs were given crib sheets as reminders, there was a marked improvement in performance, with the number of children receiving all four tests almost doubling.


Lee said most children with a high temperature had a self-limiting viral illness, which would fully resolve without any harm to the child. “However, differentiating these children from the ones who then go on to develop the serious infections, the serious illnesses, can be incredibly tricky,” she said.


“Despite everything that we are doing, sepsis still continues to be an issue, and infection is the leading cause of death for children under the age of five. There has been a really big push recently to recognise sepsis and serious infection as a medical emergency.”


Dr Ron Daniels, chief executive of the Sepsis Trust, said the failure of doctors to carry out all four checks had been implicated in several deaths, including those of three-year-old Sam Morrish from Devon and 12-month-old William Mead from Cornwall.


He said: “We are aware this is a problem and we are seeing this not only with children but with adults too. Basic observations are not being done routinely and, where observations are being done, they are not being done as a complete set. In the majority of cases, GPs will be able to make the right decision but it’s clear we are still missing some cases.”


A Health Service ombudsman’s report into the death of Sam Morrish was highly critical of the way GPs had failed to follow the Nice guidance and had not checked his breathing or heart rate.


Melissa Mead, William’s mother, said: “Two of the four basic observations were not carried out during the last consultation with the doctor before William died. Had these been done, the pneumonia and sepsis that William was suffering with would have been detected and he would have received life-saving treatment.


“I am disappointed to hear that the basics are still being ignored. Until those in frontline primary care observe the basics, life-threatening conditions will be missed and people will continue to die in preventable circumstances.”


Scott Morrish, Sam’s father, said: “Six years on from Sam’s death, it’s disappointing that these basic checks are being ignored.” He said there was a need to raise awareness among parents of the four checks alongside symptoms to look out for.


Mellor said since her 2013 report into sepsis, the government and NHS have taken steps to improve diagnosis and treatment, including new guidance from Nice and a commitment to improving awareness of the condition.


“This will be vital in order to save lives. Too many people are still dying unnecessarily from sepsis,” she said.
