Social care crisis: MPs put May under pressure to act fast
Theresa May has come under renewed pressure to take urgent action to tackle the crisis in social care, amid growing fears in Westminster that the complex task of preparing for Brexit talks is crowding out domestic policy.
The chairs of three of the most influential Commons select committees have written to the prime minister, urging her to seek a rapid cross-party consensus on the “immense challenge” of paying for health and social care in the future.
Coming as the transport secretary, Chris Grayling, prepares to meet union leaders in a bid to prevent the latest in a series of rail strikes, and as health experts warn of a looming winter crisis in the NHS, the strongly worded call underlines the sense that May’s honeymoon in No 10 is over.
The letter – sent jointly by the Conservative MP Sarah Wollaston, of the health committee, the Labour MP Meg Hillier, of the public accounts committee, and Clive Betts, also a Labour MP, of the communities and local government committee – underscores fears that pressing issues on the home front are being set aside.
“We are calling for a new political consensus to take this forward,” the letter reads. “This needs to be done swiftly so that agreement can be reflected in the next spending round.”
The MPs say any review should cover both the health and social care systems, warning that separation of the two is “creating difficulties for individuals and avoidable barriers and inefficiencies”.
May was accused of failing to grasp the scale of the challenge, after the chancellor, Philip Hammond, offered no extra money for social care in his autumn statement in November. No 10 then hastily announced that local authorities would be able to bring forward increases in council tax to meet short-term needs.
The prime minister suggested at the time that she would seek a longer-term answer, to put social care on a sustainable footing, but has so far said nothing about how she will do so.
She was widely regarded as a safe pair of hands when she gained the premiership in July, after David Cameron resigned in the wake of the EU referendum.
But ministers and civil servants are still waking up to the magnitude of the Brexit task. The resignation of Sir Ivan Rogers, the UK’s representative in Brussels, this week reinforced growing concerns about whether the government is fully prepared for what lies ahead.
A damning editorial in the right-leaning Economist news magazine, which dubbed the prime minister “Theresa Maybe” and compared her to former PM Gordon Brown, set tongues wagging among MPs.
Jon Ashworth, Labour’s shadow health secretary, said: “Brexit is going to overwhelm every government department and we won’t be able to get on with anything else. The intellectual energy will go into Brexit, the most ambitious civil servants will want to be in the Brexit departments; it will just be the focus of everything.”
He claimed May was not “philosophically or intellectually” interested in social issues such as elderly care.
The letter from the heads of the three select committees urges May to organise a review, “which should begin as soon as possible” and involve all major parties, saying that the scale of the challenge means social care will be an issue for whichever party is in power over the coming decades.
The letter concludes: “In short, the problem is widely recognised – we now need political agreement so that a solution for the long term can be found. For our part we shall do what we can to contribute to a consensus. We look forward to hearing from you.”
Chris Ham, chief executive of health thinktank The King’s Fund, said a new plan for health and social care was “long overdue”.
“For too long there has been a lack of political leadership on these issues,” he added. “We agree with the committee chairs that a political consensus that puts health and social care funding on a sustainable footing is sorely needed. Without a consensus, patients and people in need will suffer.”
The letter was also backed by Izzi Seccombe, who leads on the issue for the Local Government Association.
“Following last month’s local government finance settlement, we said there needed to be an urgent and fundamental review of social care before the spring budget, and we are pleased the select committees back this,” she said.
A government spokeswoman said more had been done under May to integrate health and social care than ever before.
“We recognise the pressures of an ageing population, which is why we recently announced almost £900m of additional funding for adult social care over the next two years,” she said.
“But, as the prime minister has made clear, this is not solely about money. That is why we are working to find a long-term, sustainable solution which helps local authorities learn from each other to raise standards across the whole system.”

Toxic air is killing thousands of us – Theresa May must act fast | Emma Howard
The verdict is in and the government is guilty. In a landmark ruling, the high court has ruled that the plans to cut fatal air pollution are not only inadequate, but also illegal. To anyone who has seen these plans, the verdict is not surprising.
“If there was a toxin in our water system that we knew was killing thousands of people every year, the government would be coordinating Cobra meetings every day,” Matthew Pennycook, the Labour MP for Greenwich and chair of parliament’s cross-party group on air pollution, said after this morning’s ruling. But when it comes to the fatal toxins in our air, the government has not been coordinating any emergency meetings. It has been moving at a snail’s pace.
Air pollution is a known killer. The UK has been in breach of legal limits on nitrogen dioxide (NO2) for years. The government’s own reports show air pollution is causing 40-50,000 deaths every year, by exacerbating respiratory illnesses, heart disease and asthma. New evidence suggests it is not only finding its way into our lungs, but into our brains too, where it may bring about the premature onset of Alzheimer’s disease. In the words of MPs from all parties, we are facing a public health crisis.
Theresa May has pledged that she will fight the “burning injustices” that entrench social inequality and blight the life chances of children from working-class families. She says that many in Westminster “don’t know what it’s like to live like this and need to be told that what the government does isn’t a game”. But perhaps the prime minister is among them.
Children know that air pollution is not a game because they are substantially more vulnerable to toxins in the air, which permanently stunt the development of their lungs. So too are people from ethnic minority backgrounds, those living in areas of higher levels of deprivation, the elderly and those with chronic health conditions. In short, they are the most vulnerable people in our society.
Yet still, the apathy persists. It has now been more than a year since the Volkswagen scandal broke, when a major car manufacturer was revealed to have been covering up air pollution through criminal activity. In the US, VW has agreed to pay $ 15.3bn to compensate consumers and mitigate some of the damage it has done to the environment. In the UK the government has failed to take legal action or compensate consumers – all while lobbying at an EU level to water down regulations that would encourage the car industry to change. Indeed, the only reason the government has an air quality plan at all is because the supreme court ordered it to draw one up 18 months ago.
So what now? Speaking at prime minister’s questions today, May promised that the government did not “doubt the importance of the issue” and would look again at the proposals. She is right, it will – because it will have to. It now has one week to negotiate with the NGO ClientEarth, which emerged victorious from the high court this morning. It will be forced to scrap its current air quality plan, which called for clean air zones in just five cities, and backed the mayor’s plans in London, but did not mandate change in major cities such as Manchester and industrial hot spots such as Port Talbot.
Any new plan is likely to implement clean air zones in many more cities and could mean that diesel cars will face charges in many city centres. But the government should ensure local authorities have proper resources to implement the zones. It is also likely the government will have to redo the flawed modelling on which its current plans are based, which fail to properly take into account the vast discrepancy between tests in the laboratory and tests on the road.
But even this is likely to be inadequate. Road transport is the number one contributor to NO2 pollution and diesel is the main culprit. Yet we currently have a tax system that incentivises consumers to purchase diesel cars, which currently make up close to half of the nation’s fleet. Research published today suggests that for central London to become compliant, the proportion of diesel cars on the road would need to be as low as 5%. The sale of diesel cars must end and the government must reform a regulator that hauls in millions from the car industry itself.
But the car industry does not have to be the villain in this story. May has the opportunity to make it the hero and deliver a boost to the government’s shiny new industrial strategy to boot. Both Norway and the Netherlands understand this – they are striving for an emissions-free industry within the decade. China has announced a boost to its hybrid industry and even Germany, the nation that invented the car, is now mooting a federal proposal for a ban on petrol and diesel by 2030.
Electrics, hybrids and hydrogen are the future of the global car industry. If the UK doesn’t move fast by delivering the incentives and infrastructure required to clean it up, we will get left behind. If the prime minister realises this opportunity, perhaps then she might really start to play the game.

Instead, keep food away from your child if they are not asking to eat. If they get hungry, give them something simple to eat like an orange or broth. The broth is much more soothing and will help them overcome their illness quicker.

Losing Weight Fast is More Than Just Calories, Carbs, and Protein
Blood Sugar, Insulin Sensitivity, and Triglycerides are three very complicated pieces of the overall health puzzle when losing weight fast. Within these three body processes you will find huge variations between one person and the next. This is part of what makes the process of losing weight so complicated.
Some of us are more catabolic and some of us are more anabolic. This makes a huge difference as to whether are food feeds muscle growth or is stored as body fat.
Some of us have high testosterone, some of us have high estrogen. Some of us are highly sensitive to insulin. Some of us don’t produce enough T4, a thyroid hormone that is important for metabolism. Some of us have low serotonin and always feel hungry and sad.
And there are many other things that we haven’t been able to cover here.
Some of this is genetically determined and it’s always going to be easier for some people’s attempts at losing weight fast than others. But that said, there are ways you can maximize your genetic potential and manage your own metabolism to give yourself a bit of a helping hand.
And this is where it makes a lot of sense to start thinking outside the limiting concept of ‘calories in/calories out’. If your daily caloric burn is lower than normal due to a very slow metabolism, then you will find yourself very hungry and struggling hard to try and get your calorie intake low enough for losing weight fast.
It would be more beneficial to try and fix that metabolism as much as possible so that you will start to respond better to the food going into your system.
The first way to do this? Visit your doctor. If you have tried and failed to lose weight consistently, then it is possible that you may have a condition like pre-diabetes or hypothyroidism. This is particularly true if you notice other symptoms such as tiredness, acne, mood swings etc. Likewise, you should consider the role that a contraceptive pill might be having. This can increase levels of estrogen which in turn can enhance fat storage. In this case, you may notice that you started gaining weight shortly after you went on the pill and in that case, coming off it could help you to more easily manage your calories.
Otherwise, there are also a few things you can do to adjust your metabolism.
Your first option is to try and control your blood sugar levels and to increase your insulin sensitivity. This is one of the first changes you need to consider when you are working on losing weight fast.
Managing Blood Sugar
The problem with high carbohydrate, low fat diets is that they massive spike our blood sugar levels on regular occurrences. And actually, this is also a broader problem with American diets in general which contain a lot of simple carbs in the form of processed snacks.
Carbohydrates are foods that include:
- Bread
- Chips
- Fruits
- Vegetables
- Chocolate
- Cake
- Soda drinks
- Pasta
- Rice
These are the closest foods to pure sugar and as such, they provide a very immediate energy source for the human body. At the same time though, this means that they digest very quickly and release the sugar rapidly into the blood via digestion. This sudden spike then causes a surge in energy, followed by a trough once the insulin has been released and once the sugar has been used or stored as fat. The spike also means that energy is more likely to be stored as fat as the body has a sudden surplus of energy and nothing to do with it!
If you have a diet that consists of lots of bread, crisps and chocolates – and especially if you eat ‘low fat’ versions of those foods – then your blood sugar level will constantly be alternating between high and low. You’ll have a sudden surge of energy, followed by a crash with lethargy and hunger pangs and the urge to snack. What’s more, is that you’ll be consistently taxing your body and over time you can ruin your sensitivity to insulin so that your body responds less efficiently to an increase in sugar and you aren’t able to properly utilize that energy.
If you already have a genetic predisposition and you follow this with a very bad diet high in carbs, then you can eventually even cause type 2 diabetes. Even if you never get to this point, you’ll find you store more fat, feel less energetic and feel hungrier a lot of the time.
Eat More Fat and Protein
Fats and proteins meanwhile take much longer to digest and they release their energy more slowly throughout the day. This provides your body with a steady stream of glucose which it can use as it goes about daily tasks and activities. You don’t get the sudden spike but you also don’t get the sudden trough. And because your body has to make use of consistent, low levels of glucose, your insulin sensitivity stays very good and you become more efficient at using the energy in your system.
Meanwhile, fats also help your body to produce a lot of other necessary hormones such as testosterone – which enhances fat loss and muscle gain when you’re in an anabolic state.
The answer here though is not to suddenly stop eating carbohydrates altogether.
All Carbs Are Not Created Equal
Carbohydrates might spike the blood sugar but they are still important for a range of other things and for providing energy. In fact, if you completely cut your carbohydrate intake then this can be enough to immediately lower your testosterone production leading to weight gain, low energy, low mood and even poor libido.
Fortunately, not all carbohydrates are made equal and some carbohydrates spike the blood more than others. In particular, you need to look at the GI rating. The ‘Glycemic Index’ essentially tells you how much sugar hits the blood stream, with 100 being the score you would give to pure glucose. The lower the GI of a select carbohydrate, the more ‘complex’ it is and the more slowly it will release it energy.
Brown, whole-grain, whole-wheat bread for example is lower GI and so are sweet potatoes. Likewise, so too are oats. If you start your day with a big bowl of oats, it should release slowly into your system and provide you with all of the energy you need to get through the day without snacking.
Generally, any carbohydrates with lots of fiber will be slowly to digest while ‘white’, ‘soft’ or ‘sweet’ carbohydrates are more likely to spike your blood sugar.

While this patient had reluctantly accepted that fasting was not an option for him, thousands of Muslims with diabetes in the UK go ahead regardless. Many will put themselves at risk of serious illness and dangerous complications by taking part in the Ramadan fast (beginning on 28 June) when they go without food, water and even medication between sunrise and sunset – despite the fact that the Qur’an makes exceptions for the sick, pregnant women, children and anyone for whom it would cause physical harm.
As an Imam working at the hospital, Diwan sees the Ramadan dilemma from both sides, and works to resolve issues both in local mosques and among the city’s health professionals.
He has also contributed to the UK’s first study on the beliefs and experiences of Muslims with diabetes during Ramadan, being carried out by Manchester University-based psychologist Dr Neesha Patel. The results, published in the journal Health Expectations, highlight the intense pressures felt by individuals with diabetes during the period, from family, culture, religion and their own conscience.
More than half the diabetics in Patel’s study still fasted; many continued to do so through a sense of obligation, the need to conform or a belief that the Qur’an demanded it. Some altered their own medication during the period of Ramadan – mostly without the advice of their GP or practice nurse. Some were put under family pressure to follow the fast, while others felt the need to conceal their decision not to fast by snacking in secret.
This problem will not go away on its own. Patel says: “Ramadan is an annual event – it is going to be with us forever. There is a large Muslim population in the UK and the level of diabetes in some of the communities is many times higher than in the UK generally. This is a big issue. For change to happen there needs to be government support.”
The UK has a population of 2.7 million Muslims, of whom 325,000 have diabetes. The South Asian population has six times the general rate of the condition. This year the holy month of Ramadan falls in the summer, and fasters in parts of the northern hemisphere will face periods without food or water that last up to 21 hours. These long periods of abstinence will feature for the next 10 years.
Fasting in itself can sometimes be beneficial to health and for many people Ramadan is an intensely spiritual time. But many health professionals are worried about a lack of formal guidance from Islamic as well as health leaders.
Vascular surgeon Dr Noreen Khan is a practising Muslim from north London, interested in fasting, who sees people with end-stage diabetes. She believes that diabetics with good glycaemic control can fast safely if GPs revise their medication at least a month before Ramadan, in some cases temporarily substituting longer-acting drugs.

GP Dr Faizan Ahmed from Moss Side Family Medical Practice in Manchester agrees there is a need for clarity. He says: “At the moment there is a social stigma in some community groups about not fasting, and the onus is very much on the individual to make a decision.”
Since 2010, his practice has invited all patients known to be Muslim for a pre-Ramadan review of their health and medication. This he described as a “watershed”, with fewer patients ending up in A&E since, and some taking the decision for the first time not to fast because of their health problems.
In the absence of national health guidelines, Diabetes UK, in collaboration with the Muslim Council of Britain, has produced culturally-sensitive material for people who want to fast, and scripts for Imams. This year the charity is sending volunteers into five largely Islamic areas during Ramadan, with the aim of reducing diabetic complications.
Maslaha – an organisation which aims to help Muslims with the dilemmas of living in a western society – has produced award-winning medical and Islamic information in partnership with Tower Hamlets primary care trust about safe fasting.
Medical training does not cover Ramadan. Trainee GP Dr Iyegbe Iredia admits that as a Catholic she was taken aback by her first patient queries about Ramadan: “I would encourage people to fast if they want to, but the advice is all a bit ad hoc at the moment. GPs need clear guidelines, and it would be good to have somewhere to send complex Muslim patients for expert advice.”
