Smartphone app could allow men to test their fertility at home

Men may soon be able to measure their own sperm count and quality at home, using a smartphone app developed by scientists.


In early tests the gadget, designed to clip onto a smartphone, detected abnormal sperm samples with an accuracy of 98%.


In more than 40% of cases where couples struggle to conceive, the underlying fertility issue is linked to sperm abnormalities, but the researchers said that social stigma and lack of access to testing meant than many men never seek evaluation.


Hadi Shafiee, who led the work at Brigham and Women’s Hospital in Boston, US, said: “We wanted to come up with a solution to make male infertility testing as simple and affordable as home pregnancy tests.”


The team put the device together using spare parts from DVD and CD drives at a total cost of $ 4.45. Using the device simply involves drawing semen into a disposable holder that is plugged into one side of the phone attachment, in a similar way to a USB. In seconds, results of the analysis are displayed on the phone’s screen.


In the study, published in the journal Science Translational Medicine, the research team recruited 10 volunteers with no formal training, including administrative assistants employed at a Boston fertility clinic. They correctly classified more than 100 semen samples using the app.

Overall, the scientists examined 350 clinic samples and were able to identify those with low sperm counts and inactive or poorly motile sperm with 98% accuracy.


John Petrozza, director of the Massachusetts General Hospital Fertility Center and a co-author, described the device as a “true game-changer”. “Men have to provide semen samples in these rooms at a hospital, a situation in which they often experience stress, embarrassment, pessimism and disappointment,” he said.


“Current clinical tests are lab-based, time-consuming and subjective. This test is low-cost, quantitative, highly accurate and can analyse a video of an undiluted, unwashed semen sample in less than five seconds.”


Allan Pacey, professor of andrology at the University of Sheffield, who was not involved in the research, said that the techniques used for sperm quality assessment have not changed significantly since the 1950s, and that even when carried out at specialist centres can be prone to errors if the laboratory worker has not had sufficient training.


“As such, the development of an easy, cheap and accurate method to evaluate the sperm present in a sample of semen would be very welcome, particularly if it could be carried out by someone without specific training and in any location,” he said.


However, he added that the smartphone device could not replicate all the tests carried out in a specialist lab and did not analyse morphology – sperm size and shape.


“For a small number of men whose sperm are badly made, and have poor morphology, it would be important to get this diagnosed correctly,” he said. “So any man who struggles with infertility for a significant length of time, say more than 12 months, should consider getting their test repeated in a specialist laboratory, regardless of what the phone app might have concluded.”


The team behind the device are planning to perform additional testing and will file for approval from the FDA, the US regulator.

Women with cancer "should be told more about fertility options"

Girls and young women with cancer in the UK are not being given enough information about the options for safeguarding their chances of having children after treatment, fertility experts have said, while describing access to such technologies across the UK as “haphazard”.


“Cancers in young people are largely treatable with good long-term survival [but] one of the most common ill effects of cancer therapy is permanent infertility,” said Melanie Davies, a consultant gynaecologist at University College London Hospitals. “If you ask people who have had cancer who are young: ‘What is the thing that bothers you most?’, top of the list is infertility.”


While men’s fertility can be preserved through freezing sperm samples, for women the options are more complex, involving the freezing of eggs, embryos or even tissue from the ovaries – although the latter is less well established and brings a number of risks, including the possibility of re-implanting cancerous cells.


But experts have raised concerns that patients, and even oncologists, are often unaware of the options, adding that there are huge variations in the availability of the technologies, storage and funding methods around the country, with funding often being granted by commissioners on a case by case basis.


“There is a very sketchy provision – in some places it is quite well organised, in other places it is not,” said Richard Anderson, professor of clinical reproductive science at the University of Edinburgh, who with Davies has co-authored an editorial on the topic, published in the British Medical Journal.


“There is an urgent need to improve information for patients, education for oncologists, and equity of funding, to overcome the barriers to more widespread use of fertility preservation in the UK,” they write.


While guidelines from the National Institute for Health and Care Excellence (Nice) have been issued, provision of fertility preservation remains patchy, they say.


“The Nice recommendation is that women [with cancer] should be aware of this and should be offered oocyte or embryo preservation, including adolescent girls,” said Anderson. “But the issue is that that doesn’t seem to have been established around the country by any means as yet.”


While data on the proportion of women with cancer who undertake fertility preservation is lacking, Davies and Anderson say the need for action is borne out by a number of surveys, together with feedback from both patients and clinicians up and down the country. “Colleagues [are] finding it difficult to get money [and] difficult to provide an adequate and funded service,” said Anderson. According to research from the UK charity Breast Cancer Care, 88% of 176 women under the age of 45 who had breast cancer said they were not referred to a fertility specialist after being diagnosed.


Valerie Peddie, senior charge nurse and fertility specialist at the Aberdeen Centre for Reproductive Medicine, agreed with the criticism. “It is absolutely fair and it is 100% accurate,” she said. “[Having children] is just part of the natural life cycle and they may be denied having children in the future if they are not given the opportunity for discussion.”


It isn’t only cancer patients who need access to information and technologies for fertility preservation, Anderson points out, noting that certain drugs and procedures used to treat conditions ranging from rheumatoid arthritis to sickle cell disease can also affect the fertility of patients. Peddie said: “It is all about quality of life and being afforded the opportunity for discussion and being given sufficient and accurate information regarding the implications, long and short term consequences [on future fertility] of any medical treatments that they might require.”


Stuart Lavery, director of IVF Hammersmith and a consultant in gynaecology, reproductive medicine and surgery, was upbeat about the provision of fertility preservation to patients.


“Our experience is that the NHS has become a lot more responsive to this issue – and we have got so busy now that we have had to set up a dedicated clinic just to see these patients urgently and get them into the system quickly,” he said, adding that the NHS had also become more supportive in funding such procedures.


Despite the progress, Lavery admitted there was room for improvement. “I think what is happening is that the oncology teams are really just finding out what is available in their local areas, and that perhaps the relationships and the referral pathways, that patient pathway, in many places may not be as smooth as it could be.”

Increase in serious IVF complications raises concerns over use of fertility drugs

Increased numbers of women suffered from a serious complication of IVF last year, according to official figures that raise concerns about the use of powerful fertility drugs.


In 2015, 60 women were admitted to hospital with severe ovarian hyperstimulation syndrome (OHSS), a 40% increase on the previous year. The condition, which is triggered when the ovaries swell up and leak fluid, is potentially life-threatening. Symptoms include abdominal swelling and pain, nausea, dehydration and blood clots in the legs.


Leading fertility doctors warned that the trend could be driven by doctors giving women stronger drugs to harvest more of their eggs, with a view to boosting the chances of a successful pregnancy.


Professor Geeta Nargund, a senior consultant at St George’s Hospital in London and medical director of Create Fertility, said: “OHSS is a preventable condition. The number should never go up, it should always go down in modern fertility practice.”


In the past decade the numbers of OHSS cases have been steadily falling as clinics have shifted towards milder treatments, but the latest figures suggest a reversal of this trend.


Nick Macklon, professor of obstetrics and gynaecology at the University of Southampton, believes the increase could be linked to recent improvements in embryo and egg freezing technology.


In theory, the ability to freeze eggs and delay transferring embryos until the ovaries have settled down should help avoid serious cases of OHSS, which can be worsened by pregnancy. However, it also provides a greater incentive for clinicians to obtain as many eggs as possible to maximise a couple’s chances of success, he said.


“This escape route may have encouraged clinicians to return to harder stimulation regimens,” said Macklon, adding that recent Human Fertilisation and Embryology Authority (HFEA) figures showing higher pregnancy rates when around 15 eggs are obtained may also have played a role.


During IVF, a woman is typically given injections of drugs designed to stimulate the ovaries into releasing eggs, which are then harvested, fertilised and either transferred back to the patient or frozen for future use. Some women are more sensitive to the drugs, however, and their ovaries are sent into overdrive, causing them to enlarge and release chemicals into the bloodstream.


Fluid from the blood vessels leaks into the abdomen, and in severe cases, into the space around the heart and lungs. Mild OHSS is common, affecting about one in three women, but the condition can result in serious health problems and -extremely rarely – women have died.


Blood tests are now used by clinics to predict who is at risk of OHSS, based on their natural levels of a hormone called AMH. “We might reasonably expect that the trend in incidence should therefore be down rather than up, so I think there is likely to be a real cause rather than statistical artefact behind the rise,” said Macklon. “The overall numbers remain very low compared with the past, but I think there is a message in these data to remain vigilant about preventing OHSS by avoiding high dose stimulation where possible.”


According to HFEA data, in 2014, 42 cases of severe OHSS were recorded, compared to 46 the previous year, but last year the figure rose to 60 cases.


The regulator classifies severe OHSS as a grade B adverse incident, one level below the most serious category, which includes the death of a patient or an embryo being transferred to the wrong woman.


Professor Adam Balen, chairman of the British Fertility Society and a spokesman for the Royal College of Obstetricians and Gynaecologists, said: “OHSS is a potentially serious complication of fertility treatment, particularly IVF. Most women will recover with pain relief; however, for severe cases, women may need to be admitted to hospital and given treatment to reduce their risk of developing blood clots, which is a serious complication of the condition.”


HFEA figures show a steady upward trend in the number of IVF cycles carried out annually, rising from 68,000 in 2014 to around 72,000 in 2015 (roughly a 6% increase), which might explain part of the increase in adverse incidents last year.

A decade ago, Lee Cowden, a primary school teacher from Surrey, was newly married and lying in bed with her husband, Iain, after a weekend away in Norfolk. “I remember sitting up in bed gasping and saying to my husband ‘I can’t breathe.’” she said. “We hadn’t been married that long and I said ‘call my mum,’ and then ‘no, call an ambulance.’”


Lee later discovered she had suffered a heart attack as a result of OHSS.


At 25 she had decided to start trying for a baby, because her diagnosis of polycystic ovary syndrome meant she would need fertility treatment. She had been prescribed stimulation drugs to induce ovulation, but when it hadn’t worked the clinic had doubled the dose.


The first sign that she was suffering OHSS was abdominal pains, but the clinic told her this was probably due to a benign ovarian cyst. In fact, her ovaries had become severely over-stimulated and this eventually led to a blood clot forming that travelled to her heart.


“I felt a stabbing pain between my shoulder blades, which I knew was a sign of a heart attack, but I thought: don’t be ridiculous, you’re 25,” she recalls.


Since then, she said, she has met many others who have suffered from milder forms of OHSS. “So many women … think overstimulation is part and parcel of treatment,” she said. “It’s not acceptable.”


Lee, now 37, made a good recovery – although she will take daily heart medication for the rest of her life – and went on to have IVF treatment using milder drugs. She conceived her daughter, Molly, who is now nearly ten. Two years later, she conceived naturally, and her second daughter, Ruby, is eight.

Boys conceived through IVF technique have lower than average fertility

Boys conceived using an assisted reproductive technique in which sperm is injected directly into the egg have lower fertility than average, scientists have found.


The results come from the first generation of boys conceived using a technique called intra-cytoplasmic sperm injection (ICSI), who are just reaching adulthood, and show that the young men have lower sperm quantity and quality than those conceived spontaneously.


ICSI is normally used to help couples conceive when the man has a low sperm count, or where there are abnormalities in the shape or movement of the sperm. The latest findings suggest that these problems tend to be passed on to the next generation – there is no suggestion that they are caused by the technique itself.


The study found that the men conceived through ICSI, who were aged between 18 to 22, had almost half the sperm concentration and half as many motile sperm (sperm that can swim well) than naturally-conceived men of a similar age.


ICSI men were nearly three times more likely to have sperm concentrations below the World Health Organisation’s threshold for “normal” fertility.


“These findings are not unexpected,” said Andre Van Steirteghem, emeritus professor at Vrije Universiteit Brussels, who led the work. “Before ICSI was carried out, prospective parents were informed that it may well be that their sons may have impaired sperm and semen like their fathers. For all the parents this information was not a reason to abstain from ICSI because, as they said: ‘If this happens ICSI can then also be a solution for our sons.’”


The findings confirm for the first time that the problems that had caused the father’s infertility – usually genetic factors – appear to be inherited by their sons.


Scientists believe these genetic flaws would be passed on regardless of whether their children were conceived naturally or using ICSI.


“These first results from the oldest group of ICSI-conceived adults worldwide indicate that a degree of ‘sub-fertility’ has indeed been passed on to sons of fathers who underwent ICSI because of impaired semen characteristics,” said Van Steirteghem.


ICSI differs from conventional IVF treatments because the sperm is injected directly into the egg, rather than many sperm being mixed together with an egg and waiting for fertilisation to occur.


In 2014, the most recent year for which figures are available, around half of all IVF cycles in the UK involved ICSI, according to a report by the Human Fertility and Embryology Authority.


Nick Macklon, professor of obstetrics and gynaecology within medicine at the University of Southampton, said: “This was always going to be a concern, and couples were counselled that sons might inherit genetic flaws that nature was trying to get rid of by rendering the man infertile. It justifies our cautious approach. It isn’t any cause for alarm.”


Prof Richard Sharpe, an expert in male reproductive health at the University of Edinburgh, said: “In showing, as a group, that the ICSI sons have starkly abnormal semen quality compared with normal, non-ICSI-derived sons, the results suggest strongly that male fertility problems severe enough to require ICSI may be inheritable.


Importantly, the results are a reminder to us that Icsi is not a treatment for male infertility, but simply a way of bypassing a problem and leaving it for the next generation to deal with – something my generation seem horribly adept at doing.”


Prof Simon Fishel, managing director of Care Fertility, said that the findings were useful but not unexpected. “Just having low semen parameters is not evidence for the requirement of ICSI or IVF technologies,” he said. “We know many men with such are indeed able to conceive naturally. More follow up studies will be required to ascertain meaningful outcomes.”

"Racist" booklet generates more controversy for Italy"s fertility day

The controversy surrounding Italy’s first fertility day, a day designed to encourage Italians to have children, was reignited on Wednesday following accusations that the cover of a booklet published by the health ministry illustrating good and bad personal habits was racist.

The image, which appeared to show people smoking marijuana, looks like it was first used in an anti-drug campaign by the US state of Arizona, according to an image published by the Phoenix New Times.


An article in La Repubblica said the contrast between the “good” and “bad” images smacked of racism and the booklet was criticised on social media outlets as being racist.


The accusation was rejected by health minister Beatrice Lorenzin.


“The photos represent a homogeneity of people, as is the multi-ethnic society in which we live,” the health ministry said in a statement. “Racism is in the eye of the beholder.”


Italy’s first fertility day is being held on Thursday.


The controversy came just weeks after the original ad campaign to promote the national fertility day, an initiative meant to raise awareness about infertility and promote reproductive health and fertility treatments, was widely derided as offensive.


In one ad, a woman was shown holding an hourglass and the campaign included the catchphrase: “Beauty knows no age. Fertility does”.


The campaign was dropped and was meant to be replaced by one that was more scientific and focused on useful information. Lorenzin is a member of the New Centre Right, a conservative coalition partner in prime minister Matteo Renzi’s centrist government. She is known for her opposition to parental rights for same-sex parents and has been an outspoken critic of surrogacy, which is illegal in Italy.

Italy woke up last week to a series of offensive images that could have been published in the 1930s, including the slogan “Beauty knows no age. Fertility does.” They were sexist, ageist – and promoted by the government and health minister Beatrice Lorenzin to publicise 22 September, which it has designated Fertility Day. On this day, people will be able to “get more information” on how to procreate and be happier, which, as the bestselling author Roberto Saviano puts it, offends “all the people who don’t have children and those who would like to have some but can’t because there is no work in Italy”.


According to the official website: “The first Fertility Day will be celebrated to attract attention to the topic of fertility and its protection,” and “to underline the danger of falling birth rates in our country”. It also says it wants to focus on “the beauty of maternity and paternity” and “medical help for those people who are having problems conceiving”. The site offers information about how Italian regions will be involved, with doctors, pharmacists and clinics available to give advice to those interested in the subject of procreation. And in four major Italian towns – Rome, Bologna, Padova and Catania – there will be events to promote fertility and discussions on the subject.

In and of itself that might not have been cause for such a furore – but what has ignited anger is not just the idea of procreation as a matter for the state to intervene in, but the images associated with it: a series of photos of pregnancy, and some troubling adverts, such as one where a woman holds an hourglass to show that her biological clock ticking away.


The reaction has been fierce, with many questioning if there is not a better way to encourage people to have children – by promoting employment policy, for example, and by helping the economy to grow more after many years of recession. Only in the last six months has Italy started to recover from a long period of economic trouble. But growth is slow and unemployment is still very high – over 40% – among the under-25s.


It’s that age group that also feels put out . One of the slogans, “reproducing is the best way for young couples to be creative”, has been roundly condemned by many newspapers including La Repubblica, La Stampa and even Il Giornale – with headlines such as “The web attacks the health minister for her slogan: We need to make more children.”


But it is the use of the images and what they say about the place of women in Italy that has offended so many people, including those who choose not to have children for their own reasons. Why should they be discriminated against? The recent law on civil unions approved by the Italian parliament a few weeks ago gave many the impression that the state had finally accepted a broader idea of the family, but now this campaign puts the emphasis back on discrimination. Women are targeted by this fertility campaign, as if they were the only people responsible for the declining birthrate that has been affecting Italy for many years.


It all sounds so similar to the fascist slogans of the 1930s, when posters on the walls incited women to give more children to the fatherland. Many cannot believe that a female minister has launched such a sexist, ageist, anachronistic campaign in a country where many other urgent problems remain to be addressed.

Britain’s dogs are becoming less fertile. Researchers who have systematically examined canine sperm over a span of 26 years say that overall sperm quality has been in decline.

Richard Lea, of Nottingham University’s school of veterinary medicine and science, and colleagues collected samples of semen from a carefully monitored population of labradors, border collies, German shepherds and golden retrievers used as stud to breed dogs intended to help the disabled. They tested 1,925 samples of ejaculate from a total of 232 different dogs at the rate of between 42 and 97 dogs every year.


And they found a drop in sperm motility – the ability to swim in a straight line – of 2.4% per year from 1988 to 1998. Even once some dogs were excluded from the study because their fertility was in some way in question, from 2002 to 2014 the scientists still measured a decline of 1.2% per year.


They also confirmed the presence of environmental chemicals known as PCBs and phthalates in the canine semen, and in testicles of dogs castrated by veterinary surgeons in the course of routine neutering treatment. These chemicals are ubiquitous, and have been linked to both fertility issues and birth defects.


At the heart of the research is not the dog, but the question of male human fertility. Repeated tests over more than 70 years have shown a downward trend in male fertility, but there has always been argument about the consistency and accuracy of the findings.


“Why the dog?” said Dr Lea. “Apart from the fact that it is a great population of animals to work with, dogs live in our homes, they sometimes eat the same food, they are exposed to the same environmental contaminants that we are, so the underlying hypothesis is that the dog is really a type of sentinel for human exposure.”


One analysis of human sperm counts examined 60 separate studies over a 50-year span. But different research teams used different techniques, and in any case the advances in laboratory equipment over many decades meant that it would always be difficult to compare like with like.


There has been much less doubt about measured increases over the decades in rates of testicular cancer, and a condition known to affect a proportion of boy babies at birth: cryptorchidism, in which the testicles do not descend normally to the scrotum.


The Nottingham canine study group resolved a number of problems of consistency. All the dogs were healthy and well cared for. The semen sampling was supervised by Professor Gary England, Nottingham’s foundation dean of veterinary science, who launched the project, and all the samples were handled by just three technicians in the 26-year study. The researchers also looked at the dog’s food, and the chemicals in the food.


The decline in canine sperm quality does not, for the moment, augur the end of the dog as a species. “It’s very unlikely” Dr Lea said. “It’s very difficult to say at what point this becomes a problem.”


The researchers saw increases in cryptorchidism in the study dogs’ pups over the years. They also saw a clear connection between environmental chemicals and declining fertility. How this might work, however, is not so clear.


“If you think about it, we are exposed to a cocktail. Who knows how many chemicals are out there and what they are doing? It gets even more complicated when you start to look at the effects of mixtures of chemicals,” Dr Lea said.


“What we have been able to do here is just to pull out ones that we know are present, and we have tested those in terms of their effects and it does suggest there is an impact. The next stage – and it is a big next stage – is trying to tease out what else is there and how those chemicals are interacting.”

But panic among young women has been increasing. The tipping point came in 2002, when US academic Sylvia Ann Hewlett published Baby Hunger, containing the unnerving statistic (that was misleading, since it only covered a tiny sample) that 42 per cent of career women had no children at the age of 40, and most deeply regretted it.

Women on internet message boards write of scaling back their careers, or having fewer children than they’d like to, because they can’t bear the thought of trying to get pregnant after 35.” Twenge had three children, all born after her 35th birthday.
