There is a crisis in mental health for young people. Services are operating in silos and they are not working for over-tested, overstressed young people. Much emphasis has been placed on teenagers with low self-esteem, with behavioural and emotional issues and how we can support them.


At Water Hall primary school in Milton Keynes, we believe in the need to identify and address these issues early to be able to implement intervention strategies as soon as possible.


Taking action early enables vulnerable children to rebuild their self-esteem and take responsibility for their emotions, behaviour and learning. The outcome will be that they re-engage with education, perform well and are confident and happy young people.


Water Hall primary serves the Lakes Estate in Bletchley, a disadvantaged area where external issues regularly affect children’s mental and emotional wellbeing. The school has used the Kaleidoscope programme for eight years. The support system enables children to forget the things worrying them at home or elsewhere when they are in school.


Seven different stages make up a Kaleidoscope session: relax, visualise, express, move, build, explore and affirm. A designated room is used for sessions for either small groups or one-to-one sessions. Interventions last six to eight weeks.


The programme is used in all classes every day. Each morning starts with a session enabling children to be in the right frame of mind to learn. Lights are low, relaxing music is played and children are taught various calming techniques that they can use anywhere.


Kaleidoscope has had an amazing impact on the children’s emotional and mental wellbeing and their learning. Exclusions have fallen, attendance and behaviour has improved, children have taken responsibility for their learning and results have shot up. Kaleidoscope works, it gives children the tools to enable them to raise their self-esteem, with the accompanying improved outcomes for the school.


Our work proves that unless the child is in the right place emotionally and mentally, learning will not take place, however good the teaching and leadership in the school.

How can health and social care work together effectively?

As the dust settles on the chancellor’s autumn statement, the future of social care remains one of the hottest topics of discussion among MPs, political pundits and practitioners alike.


Adding to the debate is the issue of how we deliver a more joined up health and social care service in a world where demand for both is increasing.


Let’s consider what an integrated health and social care system might look like. Should the cultural differences between health and social care be preserved within a closer partnership? And what role can technology play in bringing together services that have traditionally worked in parallel?


‘This shouldn’t be a land grab’


For some local authority leaders, there should be a focus on ensuring the distinct strengths, values and characteristics of the sectors are not diminished in any way.


This was one of the topics raised at a roundtable event hosted by the Association of Directors of Adult Social Services, in partnership with Capita One, where seven directors of adult social care contributed to the discussion.


Liz Bruce, director of adult social services for the London tri-borough councils, said: “The strengths and skills you have if you mix up health and social care are tremendous if we get it right. Health is really good at things that we in social care shouldn’t take the lead in. This shouldn’t be a land grab, it should be a partnership.”


Cultural shift


To build a joint working environment that meets the needs of vulnerable citizens and their families, the first step is to get a clear picture of the potential barriers to success so they can be broken down.


A Capita One survey designed to explore whether the challenges around health and social care integration were regarded as cultural or technical found 65% of local authority leaders believed the greatest challenge was the cultural shift for the people involved. Only 35% of respondents said the greatest difficulty would be bringing the systems together.


These figures reflect the wider view that when it comes to helping people adapt to a new way of working, the importance of relationships should not be underestimated. As John Powell, director of adult social services for the London borough of Redbridge, explained during the roundtable discussion: “Where integration and joint working is successful, it is because of long-standing arrangements with relationships between people in the various organisations.”


The need to cement lasting relationships between health and social care is central to Redbridge’s approach to integrating services. “We’ve gone into GP practices, starting dialogues and working together. GPs, district nurses and social workers are all working together,” Powell said. “What we’re now trying to do is build on the existing relationships we have.”


Effective technology


So, what role can technology play in enabling a more people-centric, communications-based approach to health and social care integration?


Bringing different IT systems together may not be regarded as the main barrier to achieving more joined up ways of working. However, the roundtable debate revealed services can face other challenges in this area.


Health and social care professionals may be used to working separately, so technology needs to make it easier for authorised people in each service to share data, without needing to spend additional time or effort in doing so.


David Pearson, director of adult social services at Nottinghamshire county council, said: “No matter how these systems are brought together, it is important that, for social workers and health professionals, it feels as if it is one integrated system rather than multiple systems.”


New software solutions are being developed that can ensure people working in health and social care see the same information on an individual or family they are in contact with, in real time. For local authorities, such tools can free people to work more effectively to meet the often complex needs of vulnerable citizens and their families.


But it’s not just about sharing information more effectively. The role of technology in supporting and managing people’s needs could be significant too. Devices that remotely monitor patients’ wellbeing and allow instant contact with services in an emergency are one example of how people with specific needs could get the support they require to remain in the community.


With the right steps in place, local authorities can achieve a genuine shift in culture that will enable health and social care teams to work together, without losing sight of their distinct identities and responsibilities. Technology also has an essential role to play in paving the way for a future where health and social services have the tools they need to support our most vulnerable citizens in living happier, healthier and more independent lives, for longer.

It’s been an interesting few years for mental health; what was once an ambiguous and silent illness kept out of the public perception has recently found its voice and received an unprecedented amount of attention. Numerous articles questioning the quality of treatment available in the UK and lack of government funding have dominated the media, while an increasing number of public figures campaign against stigma surrounding the subject.


But has all this attention actually made any difference? It’s all very well the government promising radical change and raising awareness of the prevalence of mental illnesses. What remains to be seen, however, is evidence from the frontline of these changes happening.


By way of an unlikely (and unlucky) series of events throughout the past decade I’m now fully acquainted with both the NHS’s physical and mental health services, receiving care for cancer, depression, anorexia/bulimia and borderline personality disorder. Having had first-hand experience of both departments, I can say with some authority that the division between the two is astounding. When my physical health was at risk, the quality and availability of care were exceptional, and I am eternally grateful to my consultants for effectively saving my life. But for the latter, I have been continually let down and disregarded.


Nowhere has this disparity been more evident than in the treatment of my eating disorder, despite the condition having a higher mortality rate than any other psychiatric illness. For the past 10 years this eating disorder has dominated my life, limiting me both personally and professionally. This is something I’ve had to deal with – for the most part – without professional help or intervention, despite the unquestionable role social and cultural factors play in the development of eating disorders. It pains me to say – not least because the NHS is a resource for which I hold the utmost respect and gratitude – that on this occasion the public healthcare system failed to perform. 


But when at 24 I discovered a suspicious lump in my abdomen and presented it to the doctor, the speed at which I was referred to specialist services and consequently treated was in complete contrast to the seemingly interminable lists I was placed on for mental health treatment. Priority of the physical over psychological was in this case irrefutable despite comparable fatality rates between the two. In light of the purported healthcare reform, how can this handling of psychological care still persist?


In recent months my bulimia and anorexia has worsened; rapid weight loss, daily binge-purge episodes and compulsive exercising led to urgent admission to an inpatient eating disorders ward. Although this admission has been one of the most trying experiences of my life I’m all too aware of how lucky I am; these beds are unbelievably limited and had one not become available at that time, my disordered and destructive behaviours – which had already resulted in osteoporosis and arrhythmia – would almost certainly have led to sudden cardiac arrest. It wasn’t until speaking to staff upon arrival that I realised the extent of this scarcity: there are currently 34 beds for the entire area inside the M25, 34 beds for an area with an estimated population of 8.5 million. I’m struggling to find any correlation between the appalling lack of resources for a mental health condition with the highest mortality rate and the government’s grand claims of turning the system around.


As grateful as I am for the treatment I’m receiving, all too apparent is the damage that the systematic privatisation of the NHS having on the service. Building work is now under way on my ward to squeeze more bedrooms into an already overcrowded living space. The results will be staff having to cater and care for more seriously unwell patients in need of round-the-clock supervision, already limited facilities (bathrooms, toilets and dining space) being stretched to accommodate them and bureaucracy taking precedence over patients’s needs.


So five years on from Nick Clegg’s speech calling for mental health reform – and after countless parliamentary promises for a change to the system in the years that followed – radical improvements remain to be seen. Stark inequality between physical and psychiatric care and the subsequent economic and social strain this puts on individuals and communities persists, while underfunded psychiatric wards are under intense strain from lack of beds and facilities to properly care for patients and provide staff with the training they need to treat such complex conditions.


Unfortunately under the current government I don’t see things changing for the better any time soon. The Conservative party’s inexorable privatisation of public services has left the NHS in a precarious position. The hospital bed from which I’m writing sits on a ward in one of London’s largest mental health centres. It is in gradual decline as more and more ground is sold to private developers, which seems symbolic of the gradual death of our once globally esteemed social welfare system.
