Black men must face the truth about prostate cancer | Linford Christie

This summer marks 25 years since one of the greatest moments of my career, winning Olympic gold in Barcelona. It often feels like the last 25 years have flown by as quickly as the 9.96 seconds it took to run the race!


A quarter of a century on and I’ve just turned 57. In my mind, I don’t feel any different to how I felt on that day in 1992. I still feel in good shape and to this day I spend more time at the track than I do in my house. However, as I’ve grown older, I’ve become much more mindful of the potential threats to my health than I ever was then.


I’ve recently started working with the men’s health charity Prostate Cancer UK and have learned some startling statistics. As a black man over 50, my risk of prostate cancer is double that of a white man the same age. One in four black men will be diagnosed with prostate cancer in their lifetime – the equivalent of one member of a 4×4 relay squad.


The prostate is an invisible gland; you can’t see it, you can’t feel it. In fact, 17% of men don’t even know they have a prostate.


If I’m being honest, until lately, I was one of the many thousands of men who knew next to nothing about the vital, walnut-size, reproductive gland inside me. More than 11,000 men die from prostate cancer every year in the UK, but over the next 10 years Prostate Cancer UK is ramping up its investment in research and has set a goal to transform the disease into one that the next generation will not fear.


Despite the odds being stacked against black men, just saying the word “prostate” within many black communities remains a massive taboo subject. Still, in 2017, the disease is simply not spoken about and, if it is, it’s in a “hush-hush, don’t tell anyone” type of way. How can it be that something that affects so many black fathers, uncles, sons and friends is continually swept under the carpet and ignored?


My health has always been important to me. When I was in the prime of my career, it was the most important thing. I was conscious of my diet and keeping myself in shape. I had to; other than a pair of running spikes, my physical and mental fitness was all I had – my career depended on it. Unbeatable; indestructible; that’s what I wanted to be, and I believed I was.


As I’ve grown older, my diet is still vital, but I’ve come to realise that no matter how well I eat or how fit I am, I am not indestructible – nobody is. But there are defence measures I can take and arming myself with knowledge is by far the most important.


If prostate cancer is caught early, more often than not, it can be successfully treated. The problem lies in the fact that when it’s in its early stages the disease is just as invisible as the gland itself – it has no symptoms.


This is why awareness of the potential threat, talking to your doctor and spreading the word among men is so important – it saves lives.


I’m currently fronting Prostate Cancer UK’s campaign, Stronger Knowing More, to get black men to face up to their risk of prostate cancer and take action. If you’re a black man, you’re not only more likely to get prostate cancer, you’re more likely to be diagnosed at a younger age.


This challenge is a marathon, not a sprint, and if we’re going to beat it we need to break down the taboos and start talking. It’s only by doing this that we can put a stop to the number of men who die from the disease every year.

Stem cells help some men with erectile dysfunction after prostate surgery

Men unable to have an erection after prostate surgery enjoyed normal intercourse thanks to stem cell therapy, scientists are to report on Saturday at a medical conference in London.


In first-phase clinical trials, eight out of 15 continent men suffering from erectile dysfunction had sex six months after the one-time treatment, without recourse to drugs or penile implants.


The positive result showed no signs of flagging during a subsequent year-long monitoring period. “As far as we know, this is the first time that a human study with a 12-month follow up shows that the treatment is lasting and safe,” said Lars Lund, a professor at Odense University hospital in Denmark, who took part in the trials.


“That is much better than taking a pill every time you want to have intercourse,” he said.


The results were promising enough to convince Danish health authorities to authorise so-called phase III “double-blind” randomised trials in which one group of men is given stem cell therapy and another placebos.


Only men recovering from prostate cancer and able to control their bladders will be enrolled in the new experiments, Lund explained.


To perform the procedure, doctors remove fat cells from a patient’s abdomen via liposuction. The cells undergo a brief treatment and emerge as all-purpose stem cells, meaning they can mutate into almost any specialised cell in the body.


“We do not cultivate the cells or change them in any way,” said Lund’s colleague Martha Haahr, head researcher and lead author of a study detailing preliminary results, published last year in EBioMedicine.


The stem cells are then injected with a syringe into the penis, where they spontaneously begin to change in to nerve and muscle cells, as well as the endothelial cells that line blood vessels. Men are under general anaesthesia while all of this happens, and are discharged from hospital the same day.


Prostate surgery is responsible for about 13% of erectile dysfunction cases. Up to 80% of men experience difficulty having sex immediately after an operation, previous research has shown.


Diabetes accounts for 40% of erectile dysfunction cases, and vascular disease another 30%. Men with diabetes would be the next target group for clinical trials, Lund said.


The results reported at the European Association of Urology conference could be an effective “therapeutic option for patients suffering erectile dysfunction from other causes,” Haahr said.


It is estimated that nearly half of men between the ages of 40 and 70 experience erectile dysfunction to some degree.


The global market for drugs treating the disorder is expected to top $ 3.4 billion by 2019.


Failure to perform sexually can also, in some men, result from relationship problems, performance anxiety or repressed homosexuality, Haahr said.

MRI twice as likely as biopsy to spot prostate cancer, research shows

Every man with suspected prostate cancer should have an MRI scan, which is twice as likely to identify the presence of dangerous tumours as the invasive biopsy used currently, say doctors.


A major trial, which could influence a change of practice in the NHS, will amount to “the biggest leap forward in prostate cancer diagnosis in decades, with the potential to save many lives”, Prostate Cancer UK said.


Researchers publishing in the Lancet medical journal have shown that an MRI picks up 93% of aggressive cancers, compared with 48% for a biopsy. The biopsy, which removes a sample of tissue for lab testing, often misses the tumour altogether.


The Prostate MRI Imaging Study (Promis), led by researchers at University College London (UCL), also showed that more than a quarter (27%) of all men with suspected cancer could avoid a biopsy altogether.


MRI scans were shown to be better at ruling out cancer, as well as identifying tumours that are not dangerous because they are slow growing and do not need to be treated. In the trial, the number wrongly diagnosed with a cancer that needed treatment was reduced by 5%.


“Prostate cancer has aggressive and harmless forms. Our current biopsy test can be inaccurate because the tissue samples are taken at random,” said the lead author, Dr Hashim Ahmed from UCL. “This means it cannot confirm whether a cancer is aggressive or not and can miss aggressive cancers that are actually there.


“Because of this some men with no cancer or harmless cancers are sometimes given the wrong diagnosis and are then treated even though this offers no survival benefit and can often cause side effects. On top of these errors in diagnosis, the current biopsy test can cause side effects such as bleeding, pain and serious infections.”


Some men suffer a life-threatening sepsis – a bloodstream infection – as a result of the standard transrectal ultrasound-guided (TRUS) biopsy.


This is the second breakthrough in prostate cancer treatment in two months. In December, UCL published a study showing that a laser-activated drug derived from bacteria at the bottom of the sea can kill prostate cancer cells without the sometimes devastating side-effects of surgery, which can leave men incontinent or impotent.


The introduction of the scan, called a multi-parametric MRI (mpMRI), into routine NHS practice is already on its way. The National Institute for Clinical Excellence (Nice), which produces guidelines for doctors on the most appropriate treatment for patients, has already launched an early review of prostate cancer diagnosis looking at the evidence from a previous trial in men with prostate cancer that has spread, and has been awaiting the results of this one.


Some hospitals are already offering MRI before any biopsy, but it will take time before it is universal. MRI machines are now in high demand for other kinds of cancer diagnosis and there will have to be special training for the radiologists who interpret the scans, which takes a high level of expertise.


Prostate Cancer UK, which helped fund the research, is now working for the introduction of MRI scans into prostate cancer units. “The current diagnostic process for prostate cancer is notoriously imperfect, so any developments which offer improvements must be adopted as a matter of priority,” said Angela Culhane, its chief executive.


“That’s why we have already been working with a range of clinical experts and professional bodies to pinpoint the potential barriers to the widespread rollout of mpMRI before biopsy and start the work of addressing them ahead of time. Now the results are formally published, we will continue to use this insight to support health professionals and commissioners to make the necessary resource and practice changes without delay. Whilst it’s clear that the rollout of mpMRI before biopsy can’t just happen overnight, it’s critical that urgent action is taken to make it available to men.”

Laser-activated drug a "leap forward" for prostate cancer treatment

A drug activated by laser light successfully destroys early prostate cancer while avoiding side-effects that commonly occur with surgery, trial results have shown.


The new technique, called vascular-targeted photodynamic therapy (VTP), involves injecting a light-sensitive drug into the bloodstream. The drug is then “switched on” by laser pulses fired through optical fibres inserted into the prostate.


Of 196 men who received the treatment, about half showed no signs of the disease two years later, compared with 13.5% of those given standard care.


Because VTP targets only prostate tumours, it does not cause the long-term problems of impotence and urinary incontinence often associated with “radical” surgery or radiotherapy.


Lead investigator Professor Mark Emberton, consultant urologist at University College London hospital, said: “These results are excellent news for men with early localised prostate cancer, offering a treatment that can kill cancer without removing or destroying the prostate.


“This is truly a huge leap forward for prostate cancer treatment, which has previously lagged decades behind other solid cancers such as breast cancer.


“In 1975, almost everyone with breast cancer was given a radical mastectomy, but since then treatments have steady improved and we now rarely need to remove the whole breast.


“In prostate cancer, we are still commonly removing or irradiating the whole prostate, so the success of this new tissue-preserving treatment is welcome news indeed.”


Currently, men with low-risk localised prostate cancer are put under “active surveillance”, which means monitoring the disease but providing no treatment unless it becomes more severe.


In the trial consisting of 413 men, participants were randomly assigned either to VTP or active surveillance.


Only 6% of the VTP group later needed radical treatment, compared with 30% of active surveillance patients. VTP treatment also doubled the average time of cancer progression from 14 months to 28 months.


The trial, reported in the Lancet Oncology journal, was conducted across 47 treatment sites in 10 European countries, most of which were performing VTP for the first time.


Emberton said: “The fact that the treatment was performed so successfully by non-specialist centres in various health systems is really remarkable.


“New procedures are generally associated with a learning curve, but the lack of complications in the trial suggests that the treatment protocol is safe, efficient and relatively easy to scale up.


“We would also expect the treatment to be far more precise if we repeated it today, as technology has come a long way since the study began in 2011.


“We can now pinpoint prostate cancers using MRI (magnetic resonance imaging) scans and targeted biopsies, allowing a much more targeted approach to diagnosis and treatment.


“This means we could accurately identify men who would benefit from VTP and deliver treatment more precisely to the tumour.


“With such an approach, we should be able to achieve a significantly higher remission rate than in the trial and send nearly all low-risk localised prostate cancers into remission.


“We also hope that VTP will be effective against other types of cancer. The treatment was developed for prostate cancer because of the urgent need for new therapies, but it should be translatable to other solid cancers including breast and liver cancer.”


The drug used, WST11, is derived from bacteria at the bottom of the ocean. To survive with very little sunlight, the bugs have evolved to convert light into energy with high efficiency. This property was exploited to develop the drug, a compound that releases destructive tumour-busting “free radical” molecules when activated by laser light.


Gerald, a man aged in his 60s from Surrey, was one of the first patients to be treated with VTP under the care of Emberton.


He said: “The treatment … changed my life. I’m now cancer-free with no side-effects and don’t have to worry about needing surgery in future. I feel so lucky to be in this position.


“I’ve met other men who had surgery – they had to stay in hospital for days whereas I could go home the next day, and one suffered from terrible incontinence which he found very distressing.


“I had some minor side-effects for a few weeks after the operation, but I’m back to normal now.”


Each year, more than 46,000 men in the UK are diagnosed with prostate cancer and 11,000 die from the disease.

Ben Stiller speaks about diagnosis with prostate cancer

The actor-director Ben Stiller has spoken about his diagnosis with prostate cancer. Speaking on the Howard Stern Show, Stiller said he was diagnosed with “immediately aggressive” prostate cancer two years ago, and has had surgery to treat it.


Appearing alongside his surgeon, Stiller, 50, said he hoped being open about his own experiences would lead to greater awareness of prostate cancer’s symptoms. He also said he now regularly urged younger people to be checked for the illness.


Stiller said of his own diagnosis: “It came out of the blue for me. I had no idea.”


After seeing a urologist, who told him he had cancer, Stiller then spoke to “a bunch of different” doctors, including Robert De Niro’s physician, eventually having surgery to remove the prostate.


Stiller reported that he is now cancer-free but is still tested regularly.


The actor-director was last seen on screen in Zoolander 2, the belated sequel to the fashion comedy. That film was released in 2016; Stiller’s credits prior to that were the 2014 comedies While We’re Young and Night at the Museum 3.


Last month, Stiller visited the UN General Assembly to present secretary-general Ban Ki-moon with a petition bearing more than 1.3 million signatures, calling on governments to act with solidarity and shared responsibility for the world’s refugees.


His upcoming films include a new collaboration with Noah Baumbach, Yeh Din Ka Kissa, and a biopic of the founder of the Chippendales. Both will be released next year.

Monitoring of prostate cancer as effective as treatment in some cases

Regular monitoring of men with localised prostate cancer offers them the same – higher than expected – survival chance after 10 years as surgery or radiotherapy, according to a major study.


The trial, the largest of its type, looked at men diagnosed with the low-risk form of the disease through a PSA (prostate-specific antigen) test and found the survival rate for all three main treatment options was 99%, compared with the 90% researchers expected.


Having surgery to remove the prostate gland – a prostatectomy – or radiotherapy halved the risk of cancer progression, compared with monitoring, to around 10%, but caused more unpleasant side-effects.


The experts behind the study, funded by the National Institute for Health Research (NIHR) and published in the New England Journal of Medicine on Wednesday, said it has significant implications for up to 30,000 men diagnosed with localised prostate cancer via a PSA test each year.


Chief investigator Prof Freddie Hamdy, from the University of Oxford, said: “The doctor will be able to say your risk of dying of prostate cancer within 10 years is very low. If you do have [radical] treatment, it does improve [the chances the disease will not spread] but you have those side-effects. The conversation now between doctors and patients will be much better informed than in the past and will have less bias than before.”


He said clinicians had a tendency to recommend the treatment they offer personally but hoped the study would change that.


For the ProtecT trial, led by researchers at the universities of Oxford and Bristol in nine UK centres, 82,429 men across the UK aged 50-69 were tested and 1,643 diagnosed with localised prostate cancer agreed to be randomised to active monitoring (545), radical prostatectomy (553) or radical radiotherapy (545) between 1999 and 2009. The research team measured mortality rates at 10 years, cancer progression and spread, and the impact of treatments reported by men.


Approximately 50% of men under active surveillance, which includes regular PSA tests, had surgery or radiotherapy later in the trial monitoring period because of fear the disease was progressing.


The researchers said a longer-term follow-up was needed to see whether reducing the spread of prostate cancer through radical treatment boosted longevity and/or quality of life.


Prostatectomy had the greatest negative effect on sexual function and urinary incontinence, and radiotherapy caused more bowel problems, although the number of men experiencing side-effects declined over time.


The researchers stressed they were not recommending a particular course of action for men with a positive PSA test.


Co-investigator Prof David Neal, from the universities of Oxford and Cambridge, said: “Clinicians don’t necessarily have the right answer. It’s important there’s a genuine partnership so men can really understand what the benefits and disbenefits are so they can take the right decision for them as an individual.”


Anne Mackie, director at Public Health England Screening, said the study provided “key information needed to manage localised prostate cancer”.


Prostate cancer is the most common cancer in men, with about 47,000 diagnoses and 10,000 deaths every year in the UK.


But there is currently no national screening programme because of issues with the accuracy of PSA tests and their potentially harmful consequences. However, the advantage is that they may catch the cancer early.


Dr Matthew Hobbs, deputy director of research at Prostate Cancer UK, said the results were good news for men diagnosed with the localised form of the disease, adding: “Many men decide against active surveillance because of the uncertainty about the impact of that choice and the anxiety it causes.”


He said more work was needed to ensure cancers are found at an early stage, pointing out that only 22% of men diagnosed when it has spread outside the prostate and abdomen survive for 10 years.

The Affects Of Stress On Your Prostate

It is well known that stress can have a big effect on your overall health and wellbeing. High levels of stress can leave you feeling run down, tired, and increase the risk of catching viruses. If you are battling a long-term illness then stress can have a big impact on how your body copes with the disease.
What isn’t so well known is the severity of the effect stress and anxiety has on prostate conditions such as prostate cancer, prostatitis and an enlarged prostate, known as benign prostatic hyperplasia or BHP. The benefits of stress management and emotional therapy on prostate health may also be surprising to many.


Stress and prostate health
High levels of stress and anxiety can impair the body’s immune system and prevent it from fighting off disease and illness. When a man is diagnosed with any kind of prostate problem they can experience increased levels of stress and anxiety resulting from worry, fear or even anger about having the problem. This can lead to the prostate disease worsening due to the immune system being unable to fight it.
It is important to recognize the role that stress and anxiety plays in contributing and causing prostate problems. It is always better to try and prevent a disease than attempting to treat it once it has already developed. Therefore, addressing stress and its causes is highly important when considering prostate health.


Stress and prostate cancer
The progression of early prostate cancer can be slowed by reducing the amount of stress in your life whilst also eating a healthier diet and exercising regularly, according to Dean Ornish MD. Ornish and his team at the University of California found that these lifestyle changes could put a stop to the progression of prostate cancer.
The study followed 93 men who had prostate cancer. Some of the men followed a primarily vegan diet, exercised regularly and took part in relaxation methods such as yoga for one hour a day and were found to score much better on the prostate specific antigen (PSA) test which was used to follow prostate cancer growth. The men who followed this healthy lifestyle noticed a 4% decline in their PSA count compared to the men who did not follow the healthier program who saw a 6% increase in the PSA count. In addition, the men who adopted the healthy lifestyle changes were less likely to require additional treatment.
A research team at Ohio State University has found a like between stress and the spread of cancer cells in many types of cancer, including prostate cancer. Their research shows that stress triggers a “master switch” gene in the body known as ATF3, which is expressed in all types of cells as a response to stressful conditions. This gene usually causes normal and benign cells to self-destruct if they have been irrevocably damaged, but the research suggests that cancer cells coax the immune system to release ATF3 in order for them to travel round the body and infect other areas. With stressful conditions being the most likely trigger for the release of this gene, the research shows that stress can cause the cancer to worsen and spread.


Stress and prostatitis and BPH
Prostatitis is an inflammation of the prostate that results in pain in the pelvis and genital area and is the cause of a variety of urinary tract symptoms. In a small percentage of cases, prostatitis can be caused by a bacterial infection with the remainder of cases mostly being the result of chronic non-bacterial prostatitis/chronic pelvic pain syndrome. Stress can have a large impact on both types of prostatitis. Stress can worsen the symptoms of bacterialprostatitis but it can actually have a role in causing non-bacterial prostatitis developing.
Some experts believe that men who suffer stress unconsciously focus their stress on their pelvic floor muscles; in the much the same way as a person may get tight back or neck muscle due to stress. Chronic tightening of the pelvic floor muscles can lead to prostatitis and so men who take steps to reduce stress in their lives can lower the risk of developing the disease. Lowering stress levels can also ease the symptoms of someone who already has prostatitis.
Stress can have an impact on men who suffer and enlarged prostate, or BPH. Symptoms such as painful urination, urinary frequency and urinary urgency can be made worse by stress and anxiety. In 2007, a study carried out by the University of Iowa found that men who reacted the strongest to mental stress had the most severe BHP symptoms. One explanation for this could be that adrenaline is released in the body as a response to stress. This adrenaline causes the urethra to narrow, making passing urine more difficult and sometimes painful.


Emotional therapy

5 Signs You Are At Risk For Prostate Cancer

Our bodies are highly sophisticated machines that need to be properly taken care of in order to function to their fullest. Most people know that an unhealthy diet, lack or exercise and your family’s medical history are risk factors for poor prostate health, including prostate cancer.


However, recent scientific research has shown us just how delicate our bodies really are, throwing out unusual risk factors for prostate cancer such as; height, where you live and the sex of your offspring. These five are risk factors for prostate cancer that we have very little control over.


1) Height


Finally some good news for those struggling with a Napoleon complex; it seems being tall isn’t always an advantage. Scientists aren’t quite sure why this is a factor in prostate health, but several studies in both the US and UK have shown a link between height and the chances of developing prostate cancer.


The studies showed that taller men had on average a 19% higher risk of developing the disease than their shorter counterparts. In addition, age offered no defense; under 65s taller than 6’3” were found to have an increased risk of developing an aggressive form of prostate cancer. Further studies are planned to determine whether growth hormones or genetics play a role in these increased risks.


2) Libido


You won’t be shocked to read that touching yourself too much doesn’t actually lead to blindness. However, it isn’t all good news; masturbation does affect prostate health. Scientists at England’s Nottingham University found that men aged between 20-35 who masturbated or were otherwise more sexually active relative to their peers had increased rates of prostate cancer. The one ray of sunshine; they found the opposite is true as we get older, with those more sexually active amongst over 50’s carrying a lower risk of developing prostate cancer.


3) Hand Measurements


If your personal physician told you that the simplest way to test for the risk of developing prostate cancer in the under-60s was to measure the index and ring finger, you might think he had lost his marbles. However, bizarre as it may sound, your doctor would be correct.


A team of researchers from Britain’s Institute of Cancer Research and The University of Warwick found that “that relative finger length could be used as a simple test for prostate cancer risk, particularly in men aged under 60”. According to the scientists, a man whose index finger is longer than his ring finger has one-third the risk of developing prostate cancer than his fellow whose index finger is the same length or shorter than his ring finger.   


Why this has any connection to prostate health is a bit of a mystery, although speculation has centered on the role of testosterone during fetal development. Relative finger length is known to inversely correlate to the levels of testosterone a child is exposed to during fetal development- the higher the levels of testosterone, the shorter the index finger. However, no direct causal link has been established and the increased risk of prostate cancer may well be caused by genetic or a combination of factors.


4) Male Pattern Baldness


The silver lining to losing your hair is that bald men are often said to be sexier than their fully-thatched counterparts. I was only recently made aware of some startling statistics: one in four black men will be diagnosed with prostate cancer in his lifetime; one in 12 will die from the disease. This is double the risk faced by white men in the UK.


Perhaps even more concerning is the fact that 86% of black men are oblivious to the heightened threat prostate cancer poses to their health, putting thousands in danger of being diagnosed at a late stage when treatment options are limited.


The prostate gland is an important component of the male sex system, but 92% of black men don’t know what it does, 62% don’t know where it is and nearly one in five is unaware he even has a prostate.


As a black man, I find these statistics worrying, especially as prostate cancer is a disease that can be successfully treated if caught early enough. Men over 50 and men with a family history of prostate cancer also face a higher than average risk of the disease.

What can we do to make sure more black men understand the added danger they face and take the necessary action that could save their lives?


Although increasing awareness is obviously a vital priority, health professionals can play a crucial role as well. Black men not only face an increased risk of prostate cancer, they are also more likely to develop the disease at a younger age. The PSA blood test is the first step men can take to identify whether anything might be wrong with their prostate; however the test is riddled with complexities and there is still a lot of confusion among GPs as to who is entitled to the test and from what age. Some remain unaware that black men face a higher than average risk and there are a number of black men who report being denied a PSA test from their GP.


The charity Prostate Cancer UK has recently produced a set of consensus statements from a panel of independent clinical experts to help GPs use the PSA test more effectively. As part of this, experts recommend that all men should be able to access PSA testing from the age of 50, but men at higher than average risk of prostate cancer (including black men) should be able to access the test from age 45.

My constituency, Streatham in Lambeth, has one of the highest black populations in the country, which is why this issue is so important to me as their MP. I want to make sure that all GPs, not only in my constituency but across the UK, help to raise awareness of the increased risk of prostate cancer in black men and have the knowledge to initiate these important conversations with the community.


Although we still don’t know why black men face a higher than average risk, my message is clear: with one in four black men being diagnosed with prostate cancer, it is up to us in the community to act. Please speak to your dads, uncles, brothers and friends and make sure every black man over the age of 45 is wise to the risk we face. Don’t let people die from embarrassment. Ignoring prostate cancer won’t beat it.
