‘Communities provide the best solutions to their own problems’ | Rachel Pugh

A year since Greater Manchester became the first region in England to take control of its £6bn health and social care budget, Jon Rouse is pleased at the progress already made, pointing to figures showing that 62-day cancer waits for Greater Manchester were among the best in the country, referrals to treatment targets are better than the England average and access to mental health services exceeds that of the north and of England as a whole. “I am proud,” he says. “In our first year, we have set up more than 20 programmes – and we are performing better overall in these areas than other parts of the country. It is quite an achievement. The key is to make sure the programmes deliver what we intend.”


As chief officer of the Greater Manchester Health and Social Care Partnership, Rouse is responsible for all the region’s health and social care that previously came under Greater Manchester’s 10 local authorities, 15 NHS trusts and foundation trusts, and 12 clinical commissioning groups. The 37 organisations are pooling their budgets to jointly organise, buy and provide health, care and support services for 2.8 million people across the region.


“Technically what we have in Greater Manchester is delegation,” says Rouse. “But what really matters is what we do. Devolution allows more flexibility on how the budget is allocated.”


Rouse has big ambitions for 2017. He wants to see further reductions in the number of unnecessary hospital admissions, GPs providing quality care to those who really need it through a new focused care programme aimed at the most deprived populations – and he wants to beat national standards on mental health.


But improving the basics are also vital, he says. “Devolution does not, however, mean that poor performance will be tolerated.” Greater Manchester has not met the 95% target for patients to be seen within four hours in A&E over the past 17 months, despite rigorous planning, including reducing hospital occupancies in the run-up to Christmas. The average in England is 85.1%, but Greater Manchester’s worst performing area, Stockport, only reached 70.5% and none of the others even reached the England average, except for Central Manchester (88.9%).


Rouse believes proposals announced this week to reform how urgent care is delivered will change all that. “We need to improve the routine GP service and out-of-hours care, but in return we need the public not to turn up to A&E when they do not need to,” he says. “For that to happen, we need to make it as easy as possible for them to know what the alternatives are. In the past we have not done that.” The plans include an app to inform the public which services are open and how busy they are – and a centralised operational hub in south Manchester, which will coordinate how hospitals respond to demand for urgent care across the region. “We are not downgrading any of our A&Es,” says Rouse. “However, we have designated four of our A&Es as higher acuity hubs.” These, which would treat the sickest patients, are Royal Oldham, Central Manchester, Stockport and Salford Royal.


An acute clinical services strategy is also under way, which aims to standardise and consolidate clinical services. Rouse says all hospitals will continue to provide general services, but would also have specialist roles for particular services. “We want to make sure that each hospital has a distinctive role, playing to its strengths. For example, at present Wigan is renowned for orthopaedics and south Manchester for coronary care,” he says.


Rouse points to the Greater Manchester cancer plan as one of devolution’s top achievements. With the exception of the Christie and Central Manchester University NHS foundation trusts, all the hospitals in Greater Manchester exceeded the 85% standard for patients to receive their first treatment within 62 days.


He admits that delayed discharges from hospital are more problematic, with 312 patients across Greater Manchester clocking up 6,729 days for January 2017 alone. “At the moment we are scrambling and it is not sustainable,” says Rouse. “Every day of every week we are being as creative as possible to find more community capacity.”


With the cumulative social care funding gap in the region predicted to reach £2bn by 2021, what can Rouse do practically to reduce this? He speaks with passion about the region’s new social care plan, launched in February, which aims to provide more support for carers, improve care home standards and encourage more people into social care through a new apprentice scheme. At least 70,000 people across the region are carers for more than 50 hours a week. If even 5% withdrew their support, it would leave an additional 3,500 people suddenly in need of the provision of full-time care from the state.


But in the end it is down to money, he says. “Integration is not the panacea. We need a new settlement in return for the way that social care is administered.” Whether Greater Manchester’s share of the £2bn extra funding announced this month is enough remains to be seen.


What about those who say “Devo Manc” is undemocratic and centralising? Rouse agrees that there was no referendum to introduce it, but he insists that there are benefits: “When something goes wrong in one part of the system, it is owned collectively. We treat it as one NHS and social care.


“Our strongest assets are our communities,” he continues. “Communities have the strength and provide the best solutions to their own problems. You have to facilitate that. I want an end to paternalism and talking shops, and to see the release of the power of the community.” It all sounds a bit Pollyannaish. What does he worry about? “There is the danger that the plans and discussions become disconnected and do not translate into changes at ground level,” Rouse admits. “My measure of success will be what people’s experience of healthcare is and whether it has improved. Devolution is not magic dust.”

Want the Best Organic Produce? Its at the Farmer’s Market

Over 2,000 years ago, the Greek physician Hippocrates said: “Let food be thy medicine and medicine be thy food.”


All who strive to eat for health know what this means: Among other things, consume more vegetables, especially green leafies and cruciferous ones like broccoli. But how do we make up for the fact that over the years, the nutritional value of vegetables and fruits has gone down significantly? How can we get what we need in the form of phytonutrients, minerals, and cancer-preventative substances in a world where even produce labeled “organic” just isn’t what it used to be?


Fifty Years of Bad Agricultural Practices


A comprehensive report published by the University of Texas in The Journal of the American College of Nutrition evaluated U.S. Department of Agriculture data for 43 different vegetables and fruits. They found that all of these produce items experienced “reliable declines” in protein, calcium, phosphorus, iron, vitamin B2, and vitamin C between the years 1950 and 1999.


The U of T researchers concluded that the reason for the decline had to do with “the preponderance of agricultural practices” which were (and still are) designed to improve characteristics such as size, length of time it takes to grow and resistance to pests but not nutritional value.


Get Thee to a Farmers’ Market!


Apart from monitoring our own soil and growing our own veggies, there isn’t much a single person or family can do about the overall state of the soil today. We can make a difference with our consumer dollars, however, while at the same time obtaining the most nutritious veggies out there and supporting our local economy.


Studies have shown that there is a big difference in nutritional value between produce that is grown commercially, picked before it has ripened, and shipped long-distances versus produce that is allowed to grow for as long as possible on the vine, tree or in the ground, then shipped just a short distance.


Although some health food stores strive to work with local farmers to bring in fresh regional produce, the “best nutritional bang for your buck” may be found at your local farmers’ market.


U.S. consumers appear to be catching on to the appeal of farmers’ markets. According to Cornell University, between 1994 and 2006, the number of farmers markets in the U.S. more than doubled. There was an estimated 3 million Americans who shopped at them and roughly 30,000 small farmers and food entrepreneurs who “earn(ed) a partial or full living selling their local produce at farmers’ markets” in 2006. That number is more than likely much larger now.


When shopping for produce at your local farmers’ market, consider these three factors:


  • Choose local and heirloom varieties whenever possible. If you shop locally, chances are you are already going to see a big difference in taste as well as nutritional content based on the factors mentioned above. There is also a huge difference in nutrient content between
    “regular” varieties of many veggies and region-specific or “heirloom” varieties. A purple/yellow carrot (a common cite at many farmers’ markets across the country) has about 38 milligrams of phytonutrients for each gram of dry weight, compared to an orange carrot, with a mere 2.5 milligrams of phytonutrients. Heirloom seeds are also among some of the last remaining seeds that are still not genetically modified.

  • Be sure that what you are purchasing really is organic. At the market, go for the produce stands that have the “Certified Organic” seal on their banners. Talk to the people working at those stands about the farm’s organic practices. Those behind the counter should be able to easily tell you what they are, and many will even invite you to visit their operation. If they are not willing to talk to you, go to another stand! Unfortunately, charlatans exist in the world of organics as well.

  • Stick around for a while to enjoy the “scene.” Let’s face it, it’s hard to be in a hurry at a farmers’ market—and that is a very good thing. These weekly events often showcase local music and ready-to-eat items you can snack on while you shop. Educational opportunities and chances to connect with your community abound at weekly farmers’ markets, so be sure to take advantage of the opportunity.

Long before there were supermarkets with their shiny aisles, there were market stands where local farmers would sell their wares. Whenever possible, get yourself and your family to a farmers’ market. Do it for your health’s sake, for your local economy– and also because it is just a lot of fun!

Study Shows That A Vegan Diet For Weight Loss Is Best

The mainstream media like to remind us that eating all that sugar will lead to weight gain and if you need to achieve weight loss you need to minimize your consumption of carbohydrates (including fruits).


Well, many people on a vegan diet, including a high fruit vegan diet, have found the exact opposite to be true.


A study from the University of South Carolina supports the fact that a vegan diet for weight loss is best, if compared to many other diets.


A Vegan Diet For Weight Loss Is Best


There was a study published in The International Journal of Applied and Basic Nutritional Sciences (1), and they compared the amount of weight lost by people on various diets.


Participants who took part in this 6-month study were eating 5 different diets, including:


  • Omnivore diet, which includes all animal products

  • Semi-vegetarian diet with occasional meat intake

  • Vegetarian diet, which excludes meat, poultry and fish, but includes dairy

  • Pesco-vegetarian diet, which excludes meat, but includes seafood

  • Vegan diet, which excludes all animal products

At the end of the six months, they concluded that a vegan diet for weight loss worked best.


Those people who were eating the vegan diet lost more weight than the other groups by an average of 4.3%, or 16.5 pounds in weight. 


Weight loss results on a vegan diet started appearing after 2 months of them following it.


The study found that weight release was not the only positive for those participants in the vegan group.


They also showed the biggest amount of decrease in their fat levels at the two and six month checks, had improved macro nutrients more than the other diets and also lower BMIs.


Eating a whole foods vegan diet with lots of fresh fruits and vegetables has many advantages, including:

Detox (detoxification) is a naturally occurring and ongoing process that happens 24 hours a day, 7 days a week. And with exposures from many types of toxins and stress in the modern world, the body has a lot of work to do.


The problem of toxicity is very real and shouldn’t be underestimated. Toxins, in large quantities due to long-term exposure, sluggish detox, stress or poor nutrition, can adversely affect the liver, brain, lungs, and kidneys.

Liver

Cat Food For Urinary Health: What’s the Best Cat Food for Urinary Tract Health?

Feline lower urinary tract disease (FLUTD) affects an estimated 1-3% of cats each year. That makes it one of the most common cat diseases. Feline urinary tract disease results in difficulty urinating or the total inability to urinate. In the case of a complete blockage, FLUTD can result in toxins building up in the body – this can lead to death. There are numerous factors that contribute to your cat’s risk of developing a UTI or other urinary tract disease. One important risk factor is their diet. Let’s take a look at different types of cat food to figure out what’s the best cat food for urinary health.


Let’s talk about dry cat food for urinary health. Unfortunately, dry cat food is the most popular type of cat food in the United States, despite the huge body of evidence that it contributes to urinary tract disease.


It all comes down to nature. Cats are obligate carnivores. They naturally subsist on raw prey. Consider that their prey consists of around 75% water. Looking at it this way, it makes sense that cats in the wild don’t spend a huge amount of time around the watering hole. When you take a cat that is biologically designed to eat animals and feed them a dry diet, a problem appears. The cat doesn’t drink enough water to make up for their dry diet. Simply put, cats that live off of a dry diet are chronically dehydrated.


The Jerusalem study, published in a 2011 edition of the Journal of Feline Medicine and Surgery, revealed something that cat owners should be paying attention to. Two groups of 82 cats, one group diagnosed with urethral obstruction and the other group healthy, were involved in the study. Of the 82 cats diagnosed with urethral obstruction, 83% consumed only dry food. In the healthy control group, the story was different: just over half of the healthy cats ate dry food. That makes sense, since dry food is the most popular variety on the market.


Dry cat food for urinary health has a very specific purpose. It features a special blend of minerals and regulates your cat’s urinary PH levels. These adjustments do work for treating certain types of urinary tract crystals. However, most cases of FLUTD are not crystal-based. Most are classified as idiopathic. In other words, most of the time, the exact cause of the disease is unknown. In most cases, this dry food is no better than the regular kind. It’s better to go for any high-moisture diet than to choose a prescription dry food.


Again, it all comes down to moisture content. No dry cat food, no matter how many veterinarians recommend it, has enough moisture to promote good urinary health.


Remember the Jerusalem study we talked about earlier? After concluding that 83% of the sick cats lived off of dry food, the remainder consumed a combination of dry and wet foods. And guess what? None of the cats suffering from urethral obstruction ate only wet food. Studies show that when cats switched to wet food after suffering from idiopathic urinary tract disease, recurrence rates were cut in half. Canned food has a lot of water in it. Moisture is the key to maintaining your cat’s urinary health.


Raw cat food is the closest thing to your cat’s biologically appropriate diet. On a raw diet, your cat will get the moisture they need while also benefitting from the balanced nutrition raw feeding provides. Overweight cats are at a higher risk for developing urinary tract disease. Raw food is exceptionally high in moisture and protein, both key factors in weight management.


The best cat food for urinary health is anything that’s not dry food, essentially. It generally doesn’t matter how expensive the food is or how many veterinarians approve of it. This rule includes dry food that claims to be good for urinary health. Dry food lacks the moisture content your cat needs to support a healthy urinary tract. As obligate carnivores, cats expect to get their hydration from their food, causing them to have low thirst drives. A dry diet leads to chronic dehydration and thus, poor urinary tract health. Cats who eat wet food have significantly lower rates of urinary obstruction and ex-dry food eaters who switch to wet food see 50% fewer recurrences of urinary tract disease.

Hospital ID badge dangling from his neck, Angus considered the empty bed in front of him. After a few strong sniffs, he moved on.


Nearing the next bed, his floppy ears perked up before he stopped dead in his tracks, tapping his paw and eyeing his handler expectantly.


The two-year-old English springer spaniel is believed to be the only canine hospital employee in the world trained to sniff out the notorious superbug Clostridium difficile, or C difficile.


The seeds of Angus’ unlikely career were planted three years ago, after Teresa Zurberg, a Vancouver resident, suffered a C difficile infection. Her bout with the bacteria – which attacks people whose immune systems have been weakened by antibiotics – left her in the hospital for five days and she lost 20 pounds.


“It was awful. I almost died,” Zurberg said.


Her husband, Markus, a nurse who works in patient safety and quality care, stumbled across an article on a beagle in the Netherlands who had been specially trained to check patients for C difficile.


Could Zurberg – a canine handler who trains bomb- and drug-detecting dogs – do the same thing in Vancouver? he wondered. “I told him, ‘If it’s got a smell, I can teach a dog to find it,’” Zurberg said. Their new beagle pup might be perfect for the job, she suggested.


The pair approached Vancouver Coastal Health, the health authority that oversees the city’s general hospital – and were surprised by the enthusiastic reaction they received. “I was expecting them to laugh at us but they were like: ‘Hey, that’s really cool.’”

Working with the health authority, Zurberg and Angus became part of a one-of-a-kind pilot program. The hope, said Nancy Desrosiers of Vancouver Coastal Health, was to find an innovative approach to address what has become a global concern.


“It really is one of those vicious cycles,” she said, pointing to a steadily rising number of infections that result in longer hospital stays and increase the likelihood of further infections. “In Canada, 64% of all of our C difficile cases are hospital-acquired.”


Unlike the patient-sniffing beagle in the Netherlands – who is now retired – Angus, it was decided, would focus exclusively on searching for C difficile in the hospital environment.


Hospitals normally use ultraviolet light to find the bacteria, but Angus can move much more quickly and efficiently through rooms. Once he detects the bacteria, the area is cleaned with a robot that uses ultraviolet light to disinfect 99.9% of the C difficile spores.


The decision to have him work on environments rather than patients minimised the chances of Angus triggering patients who may have allergies or sensitivities to dogs.


With the dog’s role in the hospital now clearly laid out, Zurberg began training Angus, mimicking the techniques used to train dogs to detect bombs or drugs.


“All of the detection work for the dogs, it’s just a game. To them, it’s just a way to get what they really want, which is their toy or their food, depending on what they’re rewarded with.”


As she taught him to associate the scent with a food reward, researchers from the hospital kept her stocked with a steady supply of the scent of C difficile – isolated from the bug – on cotton swabs.

The training took about 10 months. After passing a series of detection tests in a local nursing college with mock patient wards, Angus was brought into the hospital this summer.


Easily excitable when he’s not focused on work, Angus is accompanied by Zurberg at all times during the four days a week he spends at the hospital.


“It’s a very multi-ethnic health authority and I have to be really conscious of who’s around me and what issues they may have with dogs,” she said. “That’s part of the reason we chose a floppy-eared dog as opposed to a pointy-eared dog, because he’s not so intimidating-looking.”


Like any other member of the hospital staff, Angus faces a slight risk that he could be infected by C difficile. “He’s young and he’s not autoimmune compromised, so his risks are extremely low,” Zurberg said. His health is carefully monitored through regular veterinarian visits, she added.


His stint at the hospital has attracted global attention, and health authorities from Finland to Chile have contacted Zurberg with questions about replicating the idea. The aim, said Zurberg, is to develop a program that would train other dogs to sniff out C difficile and make them available to hospitals around the world.


Zurberg’s voice raises with excitement as she contemplates the possibility that Angus might hold the key to regaining control over a superbug that has affected so many people around the world.


“So many people come up to me and say: ‘My dad died of C diff,’ or ‘This person in my family has C diff,’ and they thank me for what we’re doing,” she said. “It opens the doors to other possibilities – what else can we have a dog detect? We’re only really limited by our imagination.”

WEDNESDAY, May 18, 2016 (HealthDay News) — Doctors have long warned parents to delay introducing certain foods to babies to decrease the risk of a potential allergic reaction, but a new study suggests that strategy probably doesn’t help. The study of about 1,400 children found that when babies were given peanuts, eggs or cow’s milk during their first year, they were less likely to become “sensitized” to those common allergy-causing foods. Being sensitized to a food means a child tests positive on a skin test. “That doesn’t necessarily mean a food allergy as such, but it indicates the child is on that pathway,” said the study’s senior author, Dr. Malcolm Sears. The goal is to reduce the risk of sensitization, which also reduces the risk of allergy, said Sears, a professor in the division of respirology at McMaster University in Hamilton, Ontario, Canada. The study’s lead author, Maxwell Tran, said this study, along with other research, “supports the paradigm shift that parents should not hesitate to introduce allergenic foods, especially cow’s milk, peanuts and eggs. This will reduce the likelihood of sensitization.” Tran is a health sciences student at McMaster. Sears reiterated: “Earlier is better. Don’t be afraid to introduce these foods.” But he added that even early introduction of a food doesn’t guarantee a child won’t eventually develop a food allergy. Tran is scheduled to present the study’s findings Wednesday at the American Thoracic Society meeting in San Francisco. Findings presented at meetings are generally viewed as preliminary until published in a peer-reviewed journal. According to Tran, previous guidelines recommended waiting a year before introducing cow’s milk and cow’s milk products, such as cheese, yogurt or ice cream. Doctors recommended delaying eggs until age 2 years, and peanut-containing products until age 3. Since other studies have hinted that earlier introduction of these foods might be beneficial instead of harmful, the researchers behind the new study looked at a sample of children already involved in a large Canadian child development trial. Sears noted that this was not a group of children that would be considered at high risk for allergies. Just over 1,400 youngsters from that study had their skin tested for allergen sensitization at 1 year. Nutrition questionnaires were completed by parents when the kids were 3, 6, 12, 18 and 24 months old, the researchers said. The researchers found that almost half the babies had consumed cow’s milk by 6 months, and the other half had milk by 12 months. Just 4 percent didn’t have milk until they were 1 year old, the study revealed. Only 6 percent of the babies had eggs by 6 months, while 76 percent had them before 12 months. And about 19 percent first had eggs after their first birthday, the study showed. Parents were much more likely to delay peanuts. Only 1 percent of the children had peanuts by 6 months, and 41 percent had peanuts introduced in their diet between 7 and 12 months. Fifty-eight percent of the children were over 1 year of age when they first had peanuts, the study found. The researchers found that early introduction of any of the allergic foods was linked to a lower risk of sensitization for that food. Giving a child egg before age 1 also reduced the odds of sensitization to any of the three tested foods, the study found. Dr. Jennifer Appleyard is chief of allergy and immunology at St. John Hospital and Medical Center in Detroit. She said, “The old train of thought was that the immune system is in flux for the first three years of life, and if exposed during that vulnerable period, food allergies might develop. But some of the old thoughts on how allergies develop and how best to treat them are changing.” She said that parents who followed that advice and waited to give their child these foods didn’t cause any allergies. “So many different things influence allergies… Things we do may affect a health outcome in some way, but it doesn’t completely control how allergies develop,” she said. There’s still a lot of research looking into the development of allergies, Appleyard noted, adding for now, early introduction of foods seems okay. Secular groups fight exclusion from AA: "The best support system in the world"

The Almighty plays a central part in the hallowed 12 steps of Alcoholics Anonymous. Six of them make reference to God, Him or Power. In one step, members vow to hand “our will and our lives” to God while another implores Him to remove their shortcomings.


Now the organisation’s religious undertone – and its utility in fighting alcoholism – has come under fire in Canada, where an atheist has lodged a human rights complaint alleging AA discriminated against him.


For more than two years, Lawrence Knight has watched his complaint snake through the Ontario Human Rights Tribunal. Earlier this year, the tribunal said the complaint raised a number of complex legal issues and recommended it go to a full hearing.


More than 23 years sober, Knight says he always found that the world of AA was an uneasy fit. “I come from a long line of humanists,” said the 59-year-old.


We Agnostics, a Toronto secular AA group created in 2010, seemed the perfect solution, offering all the support and companionship of AA but without the trappings of God. Here the 12 steps were still followed – each carefully rewritten to scrub out any mention of God or prayer.


But the group’s separation of church and sobriety didn’t sit well with some. In 2011, We Agnostics and Toronto’s other secular AA group, Beyond Belief, were delisted from the Toronto AA website and directory, in effect removing them from the city’s network of 500 weekly meetings.


The decision prompted tears and shock among the three dozen or so people who had embraced the secular groups. “It was painful. It’s shunning,” said Knight. “It was unbelievable that an organisation that can’t kick anybody out, and that prides itself on that, had kicked us out.”


Members of the secular groups – worried that their hard-won efforts at staying sober were now in jeopardy – vowed to push forward. What emerged was a parallel system of sorts, one that has today swelled to about 350 members in 12 secular AA groups across Toronto.


As the secular movement grew in numbers, Knight and others continued to push to be brought back into Toronto’s AA fold. They appealed to the city’s coordinating body, urging them to reconsider the decision. “We ran into a brick wall,” said Knight.


Frustration drove him to lodge a human rights complaint two years ago, claiming that AA had discriminated against his group on the basis of creed. While directed at Toronto’s coordinating body, the complaint also names the highest levels of the organisation in North America. “They didn’t make any decisions directly,” said Knight. “But there is something that they did do, in my opinion, and that is the fact that there have been agnostic meetings for 40 years, but they haven’t clarified anything; they didn’t have any human rights protocols in place.”


The result is a scattered approach to secularism around the world. In some places, such as New York City, secular groups are allowed to operate freely and under the umbrella of the local AA hierarchy. Other secular groups – from Des Moines to Vancouver – have been treated similarly to those in Toronto, pushed out and left to their own devices over their rejection of God.


Knight’s complaint will head to mediation in the coming weeks. To date, AA has argued to the tribunal that it is a special interest organisation, a status that affords it the right to restrict its membership.


The dispute hints at a broader question being asked among the group’s two million members worldwide as they seek to incorporate 12 steps first penned in the 1930s into modern times: How important is God and religion in AA’s quest to empower people to fight alcoholism?


Some, like Knight, argue that AA’s curative power lies not in religion, but instead in the fellowship it fosters. A growing body of research, he pointed out, now suggests that the roots of addiction are tangled in isolation and loneliness.


The argument clashes with the Toronto coordinating body, which has argued to the tribunal that a belief in the higher power of God is a bona fide requirement for groups in Toronto.


Officials in Toronto declined to comment, while a spokesperson from AA World Services in New York City said the organisation was unable to comment as the matter was before the Human Rights Tribunal.


Knight is hoping that the human rights complaint will force the organisation to definitively address the long-simmering issue. “Alcoholism kills more people than it saves. It’s killed a lot of my friends, it’s killed a lot of my family, it’s an insidious thing. And the best support system in the world is AA.”


But the strength of that support hinges on AA being accessible to all. “The point is that anybody should be able to go to a meeting and not feel intimidated, not feel forced out or that they have to believe in something that somebody else believes,” he said. “Because that’s just ludicrous in this day and age.”
